6 The undersigned, being duly sworn, deposes and says:

I am  the attorney for OR  the Plaintiff herein.
This is a matrimonial action.

The  Summons with Notice OR  Summons and Verified Complaint and the Notice
of Automatic Orders and, if the divorce action was commenced on or after January 25, 2016, the Notice
of Guideline Maintenance were personally served upon the Defendant herein,  within OR  outside
the State of New York as appears in the affidavit of service submitted herewith.

and executed an affidavit agreeing that this matter be placed on the matrimonial
calendar immediately.
