5.8 Are the crew aware of the requirements for reporting of accidents, incidents, non- conformities and
near misses?
All crew should study the near miss procedure stated in TGP 1.2.2 A2 Near miss flow chart.
 Near miss report can be made by any crew member in the Ocean Manager.
 Our KPI for near miss is 6 near miss per vessel
 Any incident can be reported to the office by creating an incident report in Ocean Manager

5.9 Are the officers and ratings aware of the requirements of the ISGOTT Ship/Shore Safety Check List
(SSSCL)?
Ship shore safety checklist is a safety agreement between the Ship and Shore. All crew must make
themselves familiar SSCL given in TSM Form 013.
Some items are marked by code R in the checklist. These items must be repeatedly verified by the officers
and crew during their deck round. All items with code A should be initialled and remark section should be
mentioned.

5.17 Are the crew aware of safe entry procedures into the pump room, compressor rooms and trunk spaces
as applicable?
Procedure for entering these spaces must be posted at the entrance and crew should make themselves
familiar with these. Generally following procedure must be followed for entering these spaces:
 The ventilation fan must be turned on at least 20 minutes prior to entry in this room.
 Prior to entry of personnel, the atmosphere in this space must be tested using fixed or portable gas detector.
 A personal muti-gas detector must be worn by at all times by personnel entering this room.
 The duty officer must be notified by radio on each occasion personnel enter these spaces and every 15 minutes
thereafter.

5.34 Are the crew aware of the general contents and location of the fire training manuals and safety
operational booklets?
Training manuals are provided in the smoke room or mess room. This contains information on use of life
saving and fire-fighting equipment of the ship.

5.35 Are the crew aware of the fixed firefighting equipment fitted?
All crew must make themselves familiar with the fixed firefighting equipment fitted on their ship. This can be
either CO2 or Foam extinguishing system. Gas tankers usually have DCP as fixed fire-fighting equipment on
deck.

5.37 Are the crew aware of the location and use of the International Shore connection?
International shore connection is usually kept at the entrance of accommodation. This is used for connecting
ship’s fire line with fire pump of shore in case if ship’s fire pump is not operational.

7.2 Is the Master & Crew aware of the name and contact details of the company security officer?
CSO is Benny Low. His contact details can be found in the TSM Poster 013 Contact List that is posted at
various locations inside the accommodation.

7.15 Are the crew aware of the company policy on the control of physical access to all shipboard IT/OT
systems?
Refer to TGP 3.4.2 IT security Section ‘3 GUIDING PRINCIPLES’
 No unauthorized access to Corporate IT Resources.
 Only specific device to be used on critical equipment like ECDIS.

8.7 Are all officers and ratings aware of the carriage requirements including emergency procedures for
the specific cargo onboard and are officers’ familiar with the vessels cargo system, including
emergency discharge arrangements?
All officers and ratings to be aware of the cargo handling procedures that include requirement of closed
loading, discharging and sampling procedures.

MSDS must be consulted before loading each cargo to find out following information for the cargo:
 Hazards of the cargo (Toxic, flammable, corrosive etc)
 Medical treatment following exposure to cargoes.
 Emergency action in case of fire or spill

TLV of common toxic gases:
 Benzene: 1 ppm
 Carbon monoxide: 25 ppm
 Hydrogen sulphide: 5 ppm

9.24 Is the crew aware of the design limitations of their anchor windlass and systems?
Deck crew and deck officers must be aware of their ship specific design limitations, such as, maximum
anchoring depth, number of shackles for each anchor and speed of lowering and heaving anchor, of the
anchor windlass and system. The information is available in the mooring & anchoring equipment plan,
available on the vessel.
