I have read and understood the Student Handbook in its entirety, containing the
institution’s rules and regulations, policies and corresponding sanctions for the
violations.

By signing this form, I confirm to abide by the institution’s policies and uphold the
integrity of the institution as its student. This shall not preclude AMA University
Online from taking other course of action in line with its concept of justice and
righteousness warranted by the circumstance.

The issuance of this Handbook likewise does not preclude AMA University Online
from making unilateral amendments as deemed necessary.
