PETITION

The undersigned petitioner, Atty. Jeremiah C. Bantigue, unto

this Honorable Court, most respectfully states that:

1. He is a Filipino citizen, single, of legal age, and a resident of

Ledesco Village Jaro, Iloilo City, copies of the picture of the petitioner
are hereto attached as Annexes “A”, “A-1” “A-2” and “A-3”;

2. He has completed her law studies at Central Philippine

University;

3. He is holding her private law office at Bantigue Law Office

located at Lot 1447 Block 8 Dongon B Bankers Village Jaro, Iloilo
City, copy of the sketch map of the office is attached hereto as Annex
“B”;

4. He was admitted to the Philippine Bar on May 20, 2021 as

shown by the Certificate of Admission to the Philippine Bar, which
contains the Petitioner’s Roll of Attorney No. 55345, a copy of the
certificate of admission is hereto attached as Annex “C”;

5. He is a member of the Philippine Bar in good standing with

Clearances from the Office of the Bar Confidant of the Supreme
Court and the Integrated Bar of the Philippines. Copy of said
clearances are herewith attached as Annexes “D” and “E”,
respectively and made an integral part of this petition;
 6. He has attached herewith and made integral parts hereof
her specimen signature and the true impressions of his dry seal as
Annexes “F”, “F-1”, “F-2” “F-3” and “G”, “G-1” “G-2” and “G-3”;

7. He has paid her Professional Tax for the fiscal year 2017
under Professional Tax Receipt 0416909, a copy is hereto attached
as Annex “H”;

8. He possesses all the qualifications and none of the

disqualifications of a Notary Public;

9. He has not applied for commission before any other

jurisdiction;

10. He undertakes to inform the Executive Judge within ten

(10) days in case of any change in the given address and to submit
her monthly report religiously;

11. He now desires to be commissioned as Notary Public for

and in the City and Province of Iloilo;

PRAYER

WHEREFORE, in view of the foregoing, it is respectfully prayed

of this Honorable Court, after due notice, publication and hearing to
grant this Petition for Appointment of Notarial Commission of the
herein Petitioner for and in the City and Province of Iloilo.

Other just and equitable reliefs are likewise prayed for.

I, Atty. Jeremiah C. Bantigue, of legal age, single, Filipino and a

resident of Ledesco Village Jaro, Iloilo City, under oath depose and
state that:

I am the petitioner in the above-entitled petition; I have caused

the preparation of the foregoing Petition; I have read the allegations
contained therein; and I certify that they are all true and correct based
on my personal knowledge and authentic records;

I further certify under oath that I have not commenced any

similar action before the Supreme Court, Court of Appeals, or before
any court, tribunal or agency concerned; and should I learn of any
similar cause of action filed before any of the aforesaid bodies, I
undertake to notify this Honorable Court within five (5) days from
notice.
