There wasn't a nurse on duty on the 11 PM (9/14) to 7 AM (9/15) shift resulting in no

medications being dispensed to me during those hours.

Wrong medicine given. Instead of being given 5/325 pain med x2, it was replaced by a white,
round, scored tablet. On the scored side was "GC/422".

There was only 1 CNA on the North wing during the 3-11 shift (Jackie).
