1.0 INTRODUCTION

Bahrain Petroleum Company BSC (Closed) vide email dated 21 st February 2012 has invited
Engineers India Limited (EIL) to carry our Failure Analysis and Risk Assessment of RVs in
LSFO Complex of BAPCO Refinery. BAPCO has selected a total of 104 RVs in LSFO
complex for the analysis. EIL’s proposal to carry out its services is as detailed below.

2.0 SCOPE OF WORK

2.1 Process analysis of consolidated data of identified RVS. The consolidated data for all RVs as
per attached Table will be furnished by BAPCO based on their data bank.
2.2 Site study of RVs w.r.t process/ engineering aspects
2.3 Re-establishing sizing/ suitability of type of RV w.r.t set pressure vs operating pressure
2.4 Analyzing / Identifying all causes which will demand functioning of RV
2.5 Likelihood rating for each cause shall be considered based on engineering
judgment/experience/historical data and “Risk Management in Refineries” paper provided by
BAPCO.
2.6 Severity Rating considering expected consequences (Qualitative Assessment) for each cause
shall be arrived at based on engineering judgment/experience/historical data and “Risk
Management in Refineries” paper provided by BAPCO.
2.7 Based on Likelihood Rating and Severity Rating, EIL shall estimate risk ranking of the
situation as per BAPCO’s Standard HAZOP Risk Ranking Matrix.
2.8 Preparation of Report (Executive Summary and recommendations will be part of Report)

3.0 APPROACH TO STUDY

Immediately after award of assignment to EIL, EIL shall appoint an experienced engineer as
Project Manager for the subject job. Bapco will consolidate the required data in the Table as
furnished by EIL based on their data bank. After receiving consolidated data, a team
comprising of Process Specialist, Risk Specialist and Instrumentation Engineer shall visit
BAPCO for Kick Off meeting and finalization of consolidated data for duration of 5 working
days. During this EIL team would interact with BAPCO Engineering team / Site Engineers
for understanding the problem faced during the pop test of RV’s.

Above team shall return to its head office and carryout the analysis for above problem and
prepare the study report utilizing inhouse engineering tools. In this regard estimated working
of 30 working days is envisaged in EIL Head office.

Thereafter above team comprising of Process, Instrumentation and Risk Specialists shall
visit BAPCO for duration of 3 days for presentation, discussion, finalisation, and submission
of final study report to BAPCO.

EIL proposes the following schedule for carrying out this study:
 Site Visit - 12th November 2011 to 23rd November 2011
 Analysis at Head Office - 24th November 2011 to 15th December 2011

4.0 BAPCO’s OBLIGATIONS

 BAPCO to ensure availability of following data:


 Process data sheets of RVs
 Instrument /Vendor Data sheets of RVs
 PFD and P&IDs which are relevant to RVs
 Equipment process data sheets which are related to RVs
 Relevant equipment layout and overall plot plan
 Performance history of RVs
 Fire and gas detector Layout
 Office accommodation and all necessary office support services during site visit.
 Local transportation
 Accommodation for EIL engineers visiting site.
 Safety gears / Safety passed required for visiting the site.

5.0 ASSUMPTIONS

EIL offer is based on the following assumptions:

 EIL’s scope of work shall be limited to Process Study part of failure analysis. Mechanical
aspect/ functionality & failure to perform on demand is not envisaged in EIL’s scope of
work. The same would be carried out by the RV vendor only.

 Consequence modeling using Phast / Phast Risk Softwares for various possible RV
failure scenarios including catastrophic failures and plotting of results is not envisaged.

6.0 THE PAYMENT SCHEDULE

6.1 EIL’s lumpsum charges for carrying out failure analysis shall be in line with the scope of
work indicated in S.No. 2.0 shall be US$ XXXXXX/= (US Dollars XXXXXXXXXXXXX
XXXXXXXXOnly).

6.2 Lumpsum charges stated above are based on the assumption that office space with facilities,
local transportation and residential accommodation shall be provided by BAPCO free of cost
to EIL for EIL engineers deployed in Bahrain.

6.3 Breakup of manhours in line with Contract No. C110044 shall be as follows :
