AFFIDAVIT OF AUTHENTICITY OF DOCUMENT

I, Maximino A. Naron, Jr. , Filipino citizen, of legal age, single), and a resident and
Parish Priest of San Isidro Parish, Tudela, Misamis Occidental, after having been duly sworn in
accordance with law, hereby depose and say:

1. That on April 19, 2017 there was called a meeting/conference for the
purpose of resolving the outstanding obligation of a certain Benjie Painagan
to Holy rosary Academy of Sapang Dalaga, Inc.;

2. That said conference was attended by seven (7) persons including myself as
the Recording secretary, refer to Annex “A” attached herewith;

3. That said Mr. Benjie Painagan was present during the conference and after
computing and resolving the outstanding debts as reflected in Annex “B”,
the debt amounted to Eighty Four Thousand, Nine Hundred Eighty Two
Pesos and Sixteen centavos (P 84,982.16) in Annex “C” for which
acknowledged by the same by affixing his signature, in Annex “C-1”
attached herewith;

4. That the same person promised to pay the aforementioned debt for P 3,000 a
month commencing on June 2017.

5. I am executing this Certification to attest to the authenticity and veracity of

all documents submitted.

6. I am executing this affidavit to attest to the truth of the foregoing, and to

serve as a supporting document for whatever purpose and for any other legal
purpose(s) it may serve.
