Lupa Street, Paco, Manila, do hereby appoint CASTILLO AQUINO & Associates Law
Offices c/o ATTY. MARK DAVID I. AQUINO and ATTY. NORBERTO CASTILLO, likewise a
Filipino and of legal age with postal address at #20B GSIS Road, Barangay Rosario,
Pasig City, as my true and legal representative to act for and my name and stead and
perform the following acts:

1. To file, represent me and act on my behalf on matters pertaining to the

annulment case; and
2. To sign verification and certificate of non-forum shopping in said case.
