AFFIDAVIT OF DISCREPANCY

I, POLICARPIO ARTHURO F. MERCADO, of legal age, Filipino and residing

at 9151 Mahogany St., Acacia, Pinagbuhatan, Pasig City, after having been sworn in
accordance with the law hereby depose and state that:

1. I am a policy holder of a life insurance issued by Sunlife of Canada (Philippines),

Inc.;

2. There appears a discrepancy in the said life insurance policy wherein my bane
was reflected as “ARTHUR MERCADO” which should have been
“POLICARPIO ARTHURO F. MERCADO” as appearing in my valid IDs and my
other official documents, presumably due to mere inadvertence on the part of the
person who caused the registration of the same;

3. For all intents and purposes, my true and correct name is POLICARPIO
ARTHURO F. MERCACO as correctly reflected in my Valid ID’s and other
official documents;

4. I hereby declare and attest that the foregoing discrepancy mentioned on

paragraph 2 and 3 refers to one and the same person;

5. I am executing this affidavit to attest to the veracity of the foregoing statements,

for all intents and for whatever legal purpose it may serve.

JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ALIE RAYMUNDO and NENITA CASTILLO, both of legal age, Filipinos
and residing at 9150 Mahogany St., Acacia, Pinagbuhatan, Pasig City and 9149
Mahogany St., Acacia, Pinagbuhatan, Pasig City, respectively, after having been sworn
in accordance with the law hereby depose and state that:

1. We personally know POLICARPIO ARTHURO F. MERCADO, who is residing

at 9151 Mahogany St., Acacia, Pinagbuhatan, Pasig City, being our long time
neighbor and acquaintance;

2. We know for a fact that the person by the name of “ARTHUR MERCADO” as
appearing in life insurance policy issued by Sunlife of Canada and the name
“POLICARPIO ARTHURO F. MERCADO” as appearing in his valid
identification cards and other official documents, refer to one and the same
person and that his true and correct name is POLICARPIO ARTHURO F.
MERCADO;

3. We are not in any way related to the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest
in this matter whatsoever;

4. We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish the real identity of POLICARPIO ARTHURO F.
MERCADO and for all legal intents and purposes it may serve.
