Subject: Monthly Accomplishment Report on the TSeKap Profiling Data Minanga

Introduction:

To address the most seen health problems of the poor, it is important for them to
be assessed thru health profiling of households under TSeKap Program. This program
aims to empower the poor families to cooperate in sustaining their good health by
utilizing different preventive measures.

Moreover, this program aims to lessen hospital admissions which causing the
families to worry for financial matters and reduce deaths due to preventable diseases.

Methods:

 Profile all 4Ps family members and the families/other person living with them.
 Physical measurements such as weight, height, waist, hips also blood pressure
were acquired from each member of the household.
 Family members were interviewed as long as 15 years old above.
 Data gathered were consolidated and reported every week.

Results:

 Total of 13 (6%) households profiled consists of 71 individuals.


 All 13 households were NHTS under CCT.
 As of the household interviewed, there were no newborns assessed.
 There are 3 infants profiled, there were no defaulters in immunization and there
were no AEFI reported. 2 were exclusively breastfed and the other is currently
utilizing breastfeeding as of the time of interview. There were no infants classified
as underweight or obese. 1 has no Vitamin A taken; referred to RHM for Vitamin
A.
 5 12-59 months were identified and profiled. 100% has completed their
vaccinations appropriate to their age. There were also no noted underweight and
stunted among the under 5 children. Mildred21
 There were 22 adolescents (10-19 y/o) profiled. No teenage pregnancy noted.
Health education regarding disadvantages of teenage pregnancy was done and
adolescent health programs were identified and explained to them. There was 1
postpartum mother identified. Postpartum mother had stopped taking FESO4
since delivery; health education was done and given FESO4 tablets (90tabs). PP
mother has complete postpartum visit and had no danger signs.
 Furthermore, there were 3 (14%) of adolescents were drinking alcohol and 1
(5%) smoking, health education regarding lifestyle modification and effects of the
vices to their health and their family members.
 Aged 15-49 were 34. 17 (50%) were identified WRA, 3 (17%) of the WRA were
unmet need. The 3 unmet need WRAs had their health education regarding other
FP methods that they can be used; referred to RHM for counselling. There were
7 WRA current users.
 There were 3 TB symptomatic; 1 had undergone CXray (PTB minimal), referred
to RHU Lab for DSSM (negative result as followed up); CXray was sent to
GenXpert, until this time, there has not yet result. Prevention of transmission was
taught to the client and also to the family members, support from the family
members was encouraged for the client. The 2 were referred to RHU for further
assessment and interventions.
 There were 18 clients identified having Non-communicable diseases, 6 of them
were undiagnosed hypertensive clienteles; Clients were referred to RHU for
further assessment, diagnosis and management. Requested the BHWs to
monitor their blood pressure regularly. There was 1 client currently taking
maintenance for HPN; the client was encouraged to have follow-up check-up at
RHU for assessment and to acquire free medicine for HPN patients.
 There were 2 proflied who were with disability. The 1 has gross deformity on foot
but was not using any assistive device and the disability doesn’t hinder daily
activities. The other one has mental illness, due to financial constraint the patient
hasn’t undergone psychiatric evaluation; referred to CVMC Psychiatric
Department for assessment and further intervention. Will follow up the family on
next visit in the barangay.
 Other significant data gathered:
o Owns a toilet 9 households (69%)
o Non-member of PHILHEALTH ≥21y/o: 3
o Individuals with ≥ 25 BMI : 1
