Indemnity Form

1. In consideration of Canon Singapore Pte. Ltd. (“CSPL”) accepting my participation in the CANON
PHOTOMARATHON 2010 on 23th OCTOBER 2010 (“Event”), I hereby

(a) undertake to indemnify, defend and hold harmless CSPL and all of its direct and indirect
officers, directors, employees, agents, successors and assigns from any and all losses,
liabilities, damages, costs and expenses (including reasonable legal fees and
disbursements and costs of investigation, litigation, settlement, judgment, interest and
penalties), death, personal injury, bodily injury, disease, illness and property damage
(collectively the “Losses”) and threatened Losses under third party claims, demands,
actions or threat of action (whether in law, equity or in an alternative proceeding and
whether groundless or otherwise) arising from or in connection with my participation in the
Event (including the breach of any third party intellectual property rights);

(b) acknowledge that that the activities of the Event may involve a certain degree of risks
which could result in Losses being suffered by myself and/or others and hereby voluntarily
accept and assume all such risks;

(c) agree to comply with all safety guidelines, requirements and instructions as stipulated by
CSPL;

(d) agree that to the fullest extent permitted by law, in no event shall CSPL or any of its direct
and indirect officers, directors, employees, agents, successors and assigns be held
responsible any Losses arising from or in connection with my participation in the Event;
and

(e) consent to CSPL using, reproducing and/or publishing the images provided by me in any
manner as CSPL may deem fit without compensation.

2. I declare that to the best of my knowledge, I do not have and am not affected by any medical
condition that could cause or arise in (directly or indirectly) any risks to myself or others during the
Event.

3. I confirm and agree that the content of this Form shall be binding on my estate, heirs, executors
and assigns.

4. All matters arising from or in connection with this Form, its validity, existence and operation shall be
determined in accordance with the laws of the Republic of Singapore and the parties agree to
submit to the non-exclusive jurisdiction of the courts of Singapore.

5. CONSENT OF PARENT/GUARDIAN*
I hereby give my consent and permission to allow my child/ward to participate in the Event
and acknowledge and agree to the content of this Form in the capacity of the
parent/guardian of the participant in the Event.
