I write to apply for medical mission assistance in the form of free medicines,
medical kits and the like from Alkem Laboratories Corporation (Philippines).

We, at the Parish of San Isidro Labrador, Cuenca, Batangas, will conduct a
medical mission on 23 September 2017. The medical mission is for the benefit of the
children and elderly people of the Municipality of Cuenca. We expect around 150
beneficiaries to come to the medical mission for free medical consultation and services
as well as free medicines. With this medical mission, we seek to aid our people who need
medical attention, but lack financial capacity for medical care.

In light of the foregoing, and considering that your company is in the field of
health care, we humbly seek your assistance in the form of free medicines, medical kits
and any other forms of assistance which your company may provide to make the medical
mission a success.
