As an OSAP recipient, you are expected to successfully complete a 60% course load (3.0 units
per term). If you have a permanent disability, you are required to pass a 40% course load (2.0
units per term). OSAP does not look at your average; academic progress is assessed on a
pass/fail basis.
You are still eligible to receive OSAP funding while on probation after providing a brief
explanation:

Tell us about the circumstances that lead to being placed on academic probation. (If
you have medical or extenuating circumstances, attach relevant documentation).
On August 29th 2017 I suffered a retinal detachment in my right eye. Due to this I had to undergo several weeks of medical procedures to save my vision. I was unable to function
properly during these weeks. I had limited range of movement and activity as I was required to wear a neck brace and keep my head upright for 24 hours a day until I was instructed
otherwise. After about 2 months of continuous checkups and various post surgery procedures, I was able to gradually able to resume a limited level of activity. And finally a regular level of
activity at the end of 2 months, while regularly attending checkups. Due to the nature of the program in which I was enrolled (Performance Acting) I was unable to participate and therefore
unable to attend classes. It is an intensely physical and participation based program and as I was unable to even attempt a moderate level of activity, I could not make any academic
progress whatsoever. A retinal detachment is an extremely serious and fragile condition and therefore I was unable to decide how to proceed with my studies as I did not know until each
checkup how long I would be unable to resume regular activities. This led to me deferring my offer of admission to the 2018/2019 academic year. Thus explaining my my lack of academic
achievement, or rather my inability to achieve academically. (I have attached my student health certificate for the 2017/2018 academic year as proof).

What steps you will take during your probationary period to ensure academic
success?
I have fully recovered from my condition while continuing regular checkups to monitor
the condition of my eye. I am now ready for academic success.

What are your current academic objectives and career goals?


My current objective is to prepare myself for academic success in my program, Performance
Acting. It is something I am incredibly passionate about. Being unable to perform to the
fullest of my academic ability for the past year has been incredibly disheartening. However, I
believe that for the coming academic year I will be able and willing to perform to the fullest of
my academic ability. This is my current and foremost objective.

If you meet progress requirements, your probation will be lifted at the end of the year. If you do
not meet progress requirements, you will be placed on a restriction from OSAP:

First instance Warning. Still eligible to receive OSAP funding after form submission.
Second instance One-year restriction
Third instance Three-year restriction
Fourth instance Five-year restriction
Fifth instance All provincial loans must be fully repaid before receiving further funding
