Therapeutic actions: a synthetic prostaglandin E1 analogue: inhibits gastric
Acid secretion and increases bicarbonate and mucus
production, protecting the lining of the stomach
Indications : w/ mifepristone as an abortifacient ; cervical ripening and labor
Induction Contraindications: w/ history of allergy to prostaglandins ; use cautiously in the elderly and with renal impairement Adverse effects: GI- nausea, diarrhea, abdominal pain GU- miscarriage, excessive bleeding Nursing Considerations: assess history of allergy to this drug, take this drug 4x a day, with meals and before bedtime , report severe diarrhea and spotting
Classification: Abortafacient, prostaglandin Therapeutic Actions: stimulates the myometrium of the gravid uterus to contract in a manner that is similar to the contractions of the uterus during labor, thus evacuating the contents of the gravid uterus Indications: termination of pregnancy 13-20 k from the 1st day of the last menstrual period, postpartum hemorrhage due to uterine atony Contraindications: w/ allergy to prostaglandin preparations, use cautiously with history of asthma Adverse Effects: CNS- headache, flushing, anxiety CV- hypotension, arrhythmias GI- vomiting, nausea GU- endometritis Nursing Considerations: allergy to prostaglandin preparations, refrigerate unopened vials, administer by deep IM injections
Classification: opioid antagonist-agonist analgesic Therapeutic Actions: acts as an agonist at specific opioid receptors in the CNS to produce analgesia and sedation but also acts to cause hallucinations and is an antagonist at mu receptors Indications: relief to moderate to severe pain, preoperative analgesia Contraindications: w/ hypersensitivity to nalbuphine, use cautiously w/ patients w/ history of narcotic abuse Adverse Effects: CNS- sedation, seating, clamminess CV- hypotesion, hypertension GI- nausea, vomiting Nursing considerations: hypersensitivity to nalbuphine, reassure patient about addiction liability
Classification: antiasthmatic, tocolytic, bronchodilator Therapeutic Actions: in low doses, acts relatively at beta2- adrenergic receptors to cause bronchodilation and relax the pregnant uterus Indications: prophylaxis and treatment of bronchial asthma and reversible Bronchospasm Contraindications: hypersensitivity to terbutaline, use cautiously w/ diabetes and CAD Adverse Effects: CNS- restlessness, anxiety, fear CV- cardiac arrhythmias, palpitations, angina pain GI- nausea, vomiting, Nursing Considerations: drug tolerance can occur w/ prolonged use, don’t exceed recommended dosage
