JOINT AFFIDAVIT OF COHABITATION

We, JAYSON B. JORDA and MARICEL L. FUENTES, both of legal age, Filipinos, both
single (living together) and residents of Brgy. SubaTabunoc, Villaba, Leyte, after having been
duly sworn to in accordance with law, do hereby depose and state that:

1. We have been living together as husband and wife under the same roof,
continuously and without any interruption, since August 201209 or a period of
more than five (5) years;

2. During our cohabitation and even until the present, there exists no legal
impediment or obstacle between us to marry each other;

3. During said cohabitation, we were blessed with threewo children, namely: SETH
RAPHAEL HARVEY F. JORDAAL JOSEPH Y. CASERA, who was born on April
7July, 2012 10, 2010 at Hinabuyan, Villaba, Leyte; JAYCEL YVONNE F. JORDA who
was born on October 6, 2013 at Villaba, Leyte; and QETSIYAH VENISE F.
JORDAJODELYN Y. CASERA, who was born on June 10, 2017September 29, 2013
at Hinabuyan, Villaba, Leyte, Philippines;

4. We are willingly and voluntarily executing this Joint Affidavit of Cohabitation to

attest to the truth of the foregoing facts and for the purposes of contracting
marriage without the need of securing the marriage license pursuant to the
provisions of Article 34 of the Family Code of the Philippines.

IN WITNESS WHEREOF, we have hereunto set our hands this 114th day of
Septembery of August, 2018 in Villaba, Leyte, Philippines.
