AFFIDAVIT OF WAIVER OF APPEARANCE

before the Regional Trial Court Branch 50 of Puerto Princesa City, Palawan;

3) However, my work is based in Manila and my employer badly needs my

services. I was only able to get a leave of absence until May 11, 2018 and am
