AFFIDAVIT
(DISOWNING A DOCUMENT ENTITLED AFFIDAVIT O NON-INTEREST TO
FILE ACTION)

I, ABC , of legal age, married, a resident of _______________, under oath

hereby depose and states that:

1. I am the manager of the 123 Enterprises, located along the National

Highway,_______________;

2. On or around 4:30 PM of APRIL 4, 2018, SPO3 BBB assigned at the

Rizal PNP arrived at my shop using the RIZAl PNP Patrol Car, caused
a commotion at my shop, her drawing her service firearm at me
while she was in her police uniform, and thereafter slapping my face;

3. The incidents that transpired were caught on and recorded in our

office CCTV, and also appropriately reported in the City of San
Fernando PNP Police Blotter;

4. On May 9, 201, I received from Sgt. NNN, a copy of an Affidavit of

Non-Interest to File Action supposedly executed by me before Atty.
MMM;

5. I was pressured that time, but I had no actual part in the preparation
of the said Affidavit;

6. I never appeared before Atty. MMM nor did he interview me before

he signed said document. It was only the party of Officer NNN who
brought said document to him and submitted same to the CSF PNP;

7. I have no intention to desist on my right to file whatever criminal and

administrative action warranted on the acts of SPO3 FFF;
8. Thus if anything should happen to me, the person with the most
reason to cause harm to me is no other that SPO3 FFF and her
cohorts , and should be investigated;
 9. I have the footage taken through CCTV of what really transpired in
my store on April 24, 2018 , starting from when the Rizal PNP Police
Mobile arrived at my store with Officer NNN as one of its passengers,
and what happened until she laid hands on me and drew her service
firearm against me, and have kept them in a safe place; and

10. With the execution of this Affidavit I hereby disown the

Affidavit of Non-Interest to file action dated May 9, 2018 and
notarized by Atty. MMM under Document No. 440, Page No. 88, Book
No. 61, and series of 2018.

IN WITNESS WHEREOF, I categorically disown that it was I who

prepared, executed and voluntarily signed the supposed Affidavit of
Non-Interest to File Action, and even if it were to be considered true,
I hereby retract all the statements made in said Affidavit regarding
my having supposedly forgiven SPO3 FFF and desisting from further
action against her.
