AFFIDAVIT OF CONSENT

I, BERNADETTE T. GUBATAN, of legal age, married to Angelo Valdez, Filipino citizen

and a resident of No. 25 San Gabriel St., Bonuan Boquig, Dagupan City, after having duly sworn
to in accordance with law, do hereby depose and state;

That my said husband and I had actually been living separately since the year 2000 the
year he went to the United States of America being an American citizen and lived with another
woman up to the present;

That sometime in the early part of year 2002, ERICK I. RAMOS and I had a love affair
and lived together as husband and wife for a while resulting to the birth of RALPH LOUIE T.
GUBATAN and RALPH VINCENT T. GUBATAN who were both born as twins on February 11,
2003;

That immediately after their birth, our said twin children have been under the care and
custody of Erick’s elder sister, DRA. LETICIA I. RAMOS up to the present;

That I am personally aware that Dra. Leticia I. Ramos have been giving all her time,
attentions, care and support towards my said two sons and all their needs included;

That considering that Dra. Ramos have shown her true love and affections as a real
mother to my said twin sons, and having signified her intention to legally adopt them as her own
children, I am giving my whole heart consent thereof considering further that she is also their
aunt by blood;

That this consent is fully given without any intimidation nor promise of reward
whatsoever but for the best benefit and welfare of RALPH LOUIE T. GUBATAN and RALPH
VINCENT T. GUBATAN;

IN WITNESS WHEREOF I have hereunto set my hand this 4th day of November, 2004 at
Baguio City.
