DIRECT OBSERVATION PROCEDURAL SKILLS (DOPS)

Skill: Laryngeal Mask Airway (LMA)

Objectives:
1. To choose the appropriate size of a laryngeal mask airway.
2. To prepare and to ensure that the laryngeal mask airway is functioning well.
3. To demonstrate the correct technique for inserting a laryngeal mask airway.
4. To confirm that the laryngeal mask airway has been inserted correctly.

S - suction, stylet, securing tape, syringe,
stethoscope
Choose the appropriate size (assessor to tell the
various sizes to the student).
Inflate the cuff with a 20ml syringe following the
manufacturer’s guideline to ensure its integrity -
do not overinflate.

Detach the syringe to ensure that cuff remains


inflated.
Deflate the cuff and detach the syringe from the
pilot balloon.
Lubricate only the posterior surface of the cuff.
2. Technique Ensure manikin’s head is at the appropriate level
of the operator and is at the end of the bed.
Pre-oxygenate the manikin using the bag-valve-
mask with oxygen at a flow rate of 15L/min for 3
minutes (assessor to tell the student).
Open the mouth.
Hold the LMA with one hand and place the
forefinger of the other hand on the junction
between the tube and the mask.

Introduce the LMA into the pharynx following the
natural curvature of the pharynx and advance it
blindly until resistance is felt.
Inflate the cuff.
Connect the LMA to the bag-valve unit.
Confirm that the LMA is in-situ by ensuring
- presence of chest rise
- presence air entry
- capnograph is normal (if available)
- ability to ventilate
- the black line indicator faces the operator
Insert a bite block.
Secure the LMA using a tape.
3. Post- Clear all clinical wastes and equipment
procedure appropriately.
Remove gloves and face mask, dispose in
appropriate bin and wash hands.
Documentation.
