JUDICIAL AFFIDAVIT

Purpose of the testimony: Plaintiff, FROG PRINCE is presenting his

testimony, to prove that defendant PRINCESS TIANA personally
contracted an obligation with the plaintiff and such obligation was never
fulfilled.

FROG PRINCE, of legal age, Filipino, a resident of 143 River Valley,

Fairy Tale Kingdom, Manila, after having been sworn in accordance with
law and fully conscious that I do so under oath and that I may face criminal
liability for false testimony or perjury on way of answers to the questions
propounded to be by Atty. Gay Gonzaga from Magaling Law office , with

office address at 2nd Floor ABC Building, Sampaloc, Manila, do hereby
depose and state:

Q1: Please state your name and other personal circumstances?

A1: I am Frog Prince, of legal age, Filipino citizen, single and
with postal address at No. 143 River Valley Fairytale Kingdom,
Manila City, Metro Manila.

Q2: Are you to the Plaintiff in this case?

A2: Yes I am, Ma’am.

Q3: Do you know the defendant in this case by the name of

Princess Tiana?
A3: Yes, ma’am.

Q4: How do you know the Defendant?

A4: I knew her, because we entered into an Agreement last July
10, 2017 for the recovery of the ball in the River Valley. In
addition we’ve met several times since I was there in the pond
when the ball fell on the river.

Q5: Mr. Frog Prince, you mentioned that you are in that pond when
Princess Tiana’s ball fell on the water. May I ask the reason
why?
A5: As a frog, I have to stay nearby the pond most of the time for
my own survival.

Q6: In your stay nearby the pond, how many times have you
encountered the presence of Princess Tiana?

A6: Several times. When she always playing with that ball.

Q7: What happened after the ball fell?

A8: Princess Tiana approached me and asked me to fetch it for

her since the ball fell on the deepest section of the pond, as a
result, we executed the contract, where we both agreed that
once I recover the ball, she will kiss me.

Q5: I am showing to you a contract dated July 10, 2017 bearing

your name and with a signature above it. Are you familiar with
this contract?

A5: Yes sir, that’s the contract that Princess Tiara and myself
signed before I fetched the ball.

Q6: What did you after the contract?

A6: I immediately recover the ball, It was very dangerous but I

never hesitated, when I recovered it, I immediately went to her,
and delivered it.

Q7: It says here in the contract that you shall receive a kiss from
the Princess after you have returned the ball to her. Was this
contract fulfilled?

A7: No.

Q8: Have you demanded that kiss from the Princess thereafter?

A8: Yes madam. I even sent to her the demand letters dated July
20, 2017 reminding her of her obligation. It was evidence by
the demand letter itself, and a registry return card from
Fairytale Kingdom post office.

Q9: I am showing to you a demand letter dated addressed to the
defendant which was received on July 20, 2017 Is this the
demand letter that you have mentioned?

A9: Yes. Madam.

Q10: What did you do after sending the demand letter, if any?

10A: Since, I did not received a reply to the demand letter, I

consulted a lawyer, and hired a counsel from Magaling law
office to obliged Princess Tiana to perform her obligation.

Q11: What relief do you ask from this Honorable Court?

A11: I asked to this Honorable Court to order the defendant to

perform her obligation pursuant to the contract, and to pay me
the amount of Php25, 000 as attorney’s fee and Php100,000
for causing danger to my life for the recovery of the ball and
by way of moral damages;

Q12: I have no further questions for you. Do you have anything to

add or omit in your judicial affidavit?
A12: None Ma’am.

14Q: Will you voluntarily sign your affidavit?

14A: Yes, Ma’am.

him regarding the latter’s answers.
