Interested applicants must complete the following requirements: (1) Properly accomplished

Application Form (CS Form No. 100, Revised September 2016, available at any CSC
Regional/Field Office, and downloadable from the CSC website www.csc.gov.ph); (2) Four
pieces of recently taken (not more than three months ago) passport sized (4.5 cm x 3.5 cm) I.D.
pictures in white background, showing the applicant’s bare face (without eye glasses or any
accessory that may cover facial features), and with hand-held and written name tag legibly
showing the applicant’s signature over printed full name; (3) Original and photocopy of any of
the following I.D. cards which must be valid (not expired): Driver’s License, Passport, PRC
License, SSS I.D., GSIS I.D. (UMID), current Company/Office I.D., current School I.D., Postal
I.D., BIR I.D., PhilHealth I.D., Barangay I.D., Voter’s I.D., Police Clearance/Police Clearance
Certificate, or NBI Clearance; and (4) If the I.D. card has no indicated date of birth, original and
photocopy of birth certificate issued by the Philippine Statistics Authority (formerly National
Statistics Office), or the Local Civil Registry printed on security paper.

The CSC stressed that applicants must personally submit their applications at the CSC Regional
Office (CSC RO), or at any of the CSC RO’s Field Offices where they intend to take the
examination. Examination fee for both levels of examination is PhP500.

The second conduct of CSE-PPT next year is slated August 12, 2018. Application period shall
run from April 10 – June 22, 2018.

APPLICATION REQUIREMENTS

Below are the specific requirements the applicant must comply:

1. APPLICATION FORM: Fully accomplished application form (CS Form No. 100, Revised 2015).
Download the form here. The 'Signature' and the 'Right Thumb mark' as per CSC shall be left
blank as this will be accomplished in the presence of CSC representative/ processor.
2. ID PICTURES: Four (4) pieces of ID photos printed in good quality photo paper, Passport size,
colored with white background, with handwritten name tag showing the signature over printed
full name format. In the photo studio, just tell that the photo is to be used for Civil Service
application, they already know the specifications.
3. VALID ID with clear picture and Date of Birth and signature: Original and photocopy of any of
the following: Driver's license, PRC License ID, SSS ID, GSIS ID, Voter's ID, BIR ID, Philhealth ID,
Postal ID, Current company ID, Current School ID, Police Clearance, Barangay ID.
4. NSO BIRTH CERTIFICATE: if the ID of the applicant doesn't contain the birth date, he/she shall
bring the authenticated birth certificate.
5. EXAMINATION FEE: 500 pesos
6. In case of dual citizenship under RA 9225, the original and photocopy of Certificate of Retention/
Re-acquisition of Philippine Citizenship issued by BIR is required.

WHO ARE QUALIFIED FOR THE GRANT OF EDPSE

The EDPSE shall be conferred on passers of the proficiency test, or training course conducted by
the Information and Communications Technology Office [ICTO] (formerly National Computer
Center) on the following: Systems Analysis and Design; and Computer Programming courses of:
Java, MS Access, or Visual Basic;

APPROPRIATENESS OF EDPSE
The EDPSE shall be considered appropriate only to positions for which the eligibility was given,
to other functionally related positions belonging to the Information Technology/Management
Information System Group, such as Data Encoder, Data Machine Operator, Auxiliary Machine
Operator, Data Encoder-Controller, Computer Programmer, Information Systems Analyst, and to
other positions as may be determined by the Commission. (Provided under Item No. 8, Part V of
CSC MC No. 12, s. 2003 re: Revised Policies on Qualification Standards)

DATE OF EFFECTIVITY OF EDPSE

The date of effectivity of the EDPSE shall be the date of approval of an application by the CSC
Regional Office concerned.

PERIOD OF FILING OF APPLICATION FOR EDPSE

Filing of applications for the grant of EDPSE shall be within three (3) years from the date of
issuance of the Information and Communications Technology Office (ICTO) / National
Computer Center (NCC) Certificate of Proficiency.

DOCUMENTARY REQUIREMENTS

General Documentary Requirements

1. Properly accomplished Application Form (CS Form 101-A, Revised Dec. 2011);

2. Three (3) pieces of identical I.D. pictures taken within the last three (3) months prior to filing
of application with specifications, as follows:

a. Passport size (4.5 cm x 3.5 cm)

b. Printed on quality photo paper;

c. In white background;

d. In standard close-up shot (from shoulder level up with the head and face occupying at least
80% of the picture and with the name tag positioned at approximately 1” below the chin);

e. In bare face (without eye glasses/colored contact lens, or any accessory that may cover facial
features; facial features not computer-enhanced);

f. Showing left and right ears; and

g. With full name tag:

- The name tag must legibly show the applicant’s signature over complete printed name in the
format First Name-Middle Initial-Last Name-Extension Name (if any); and
- The name tag should not be computerized, meaning, the applicant should have his/her picture
taken while holding his/her written name tag.

Note: Any other I.D. card NOT included in the above list shall NOT be accepted. Alumni,
association, membership, and health I.D.s, including ATM cards, shall NOT be accepted.

4. Original and photocopy of Birth Certificate of the applicant authenticated/issued by the


National Statistics Office (NSO);

Note: In case where the NSO-issued Birth Certificate is not legible, or the NSO has duly issued a
Negative Certification of Birth (NSO CRS Form No. 1) printed in NSO security form, the
applicant shall, in addition, be required to submit the original and photocopy of his/her Birth
Certificate that is authenticated/issued by the Local Civil Registrar (LCR).

5. For female married applicants, original and photocopy of Marriage Certificate


authenticated/issued by the NSO;

Note: In case where the NSO-issued Marriage Certificate is not legible, the applicant shall, in
addition, be required to submit the original and photocopy of his/her Marriage Certificate that is
authenticated/issued by the Local Civil Registrar (LCR).

6. Certification executed by the applicant that he/she has no pending administrative and/or
criminal case before any court/authorized body, and that he/she has never been found
guilty/convicted of any administrative offense and/or crime, using the prescribed CSC SPEL
Form 1, April 2012; and

7. If filing of the application is through a representative:


a. Authorization letter executed by the applicant; and

b. Original and photocopy of at least one (1) valid I.D. card of the representative as listed under
Item no. 3 hereof.

Specific Documentary Requirements

a. For Training Course

1. Original and certified copy of the Certificate of Proficiency issued by the Information and
Communications Technology Office [ICTO] (formerly National Computer Center) on the
following computer courses: Systems Analysis and Design; Computer Programming; Java; MS
Access; or Visual Basic;

2. Original and photocopy of the Certificate of Completion issued by ICTO; and

3. Original and photocopy of the Grade Slip issued by ICTO.

b. For Proficiency Test

1. Original and certified copy of the Certificate of Proficiency issued by the Information and
Communications Technology Office [ICTO] (formerly National Computer Center) on the
following computer courses: Systems Analysis and Design; Computer Programming; Java; MS
Access; or Visual Basic; and

2. Original and photocopy of the Notification Slip issued by ICTO.

WHERE TO FILE APPLICATIONS FOR EDPSE

Properly accomplished (CS Form 101-A (Revised, Dec. 2011), together with the pictures and
other documentary requirements, must be submitted to the CSC Regional Office concerned, or to
any of its Field Offices.

MODES OF FILING OF APPLICATION FOR EDPSE

A. By the Applicant in Person;

This mode of filing an application involves the applicant himself/herself going to the CSC
Regional Office concerned, or to any of its Field Offices, to personally submit his/her application
and documentary requirements. This is the most recommended mode of application filing, as this
will allow interview of the applicant as to his/her qualifications and other circumstances relevant
to his/her application for eligibility grant.

B. Through Representative;
This mode involves a representative of the applicant going to the CSC Regional Office
concerned, or to any of its Field Offices, to submit the application and documentary
requirements.

Filing of application through a representative shall require the submission of the following
additional documents:

- Authorization Letter executed by the applicant; and

- Original and photocopy of at least one (1) valid I.D. card of the representative.

Note: While filing of application may be coursed through a representative, the applicant shall be
required to appear personally before the CSC for the proper issuance and acceptance of the
Certificate of Eligibility, should the application be approved. Representative/s shall not be
allowed to receive the Certificate of Eligibility on behalf of the applicant-grantee.

C. Through Conduit CSC Regional Office (RO); and

This mode involves applicants who have moved to or are presently based in another region that
is far-off from the authorized CSC Regional Office (RO) having jurisdiction over their cases.

In this instance, an applicant concerned, or his/her representative, may file the application to the
CSCRO nearest the applicant’s present place of residence or work. Said CSCRO shall act as
conduit between the applicant and the authorized CSCRO in facilitating acceptance of the
application, and release of the corresponding Certificate of Eligibility (COE) should the
application for eligibility grant be approved.

This mode of application filing takes time, considering that communications between the conduit
CSCRO and the authorized CSCRO are dispatched at least via registered mail.

D. Through Mail

This mode involves the applicant sending his/her application and documentary requirements
(including originals) to the CSC Regional Office concerned through either registered mail, or
courier. This mode takes time in view of anticipated exchange of communications.

Delays and lost of documents through the negligence of the courier is beyond the
control of the CSC Regional Office concerned.

SCHEDULE OF FEES

The schedule of fees for the grant of EDPSE, which covers two (2) stages, is, as follows:

Eligibility

Applications filed at the CSC Field Office shall be charged only the Evaluation Fee
considering that the applications will be forwarded to the CSC Regional Office (RO) for review
and final evaluation, and processing of Certificate of Eligibility.

Applications filed through Conduit CSC Regional/Field Office or through Mail (registered
mail, or courier) should enclose a Postal Money Order borne in the name of the CSC Regional
Office concerned in the amount of Php200.00 representing payment for the Evaluation Fee.

Specialist Eligibility (STSE)

Eligibilities on Scientific and Technological Specialists on the Bases of Their Qualifications and
the Requirements of Public Service”.

WHO ARE QUALIFIED FOR THE GRANT OF STSE

The STSE is conferred on an S&T Specialist who has at least a Bachelor’s degree in areas
enumerated in Section 1 of the Rules and Regulations Implementing the Provisions of PD No.
997 (Revised 2009) and who meets any of the following additional requirements:

- At least three (3) years of continuous experience in research and/or teaching in the pertinent
field, which may include specialized training in research, or teaching experience at the college
level in one’s major field or field of specialization, or completion of a patented invention; or

- Has earned a master’s or doctorate degree in any of the fields of study enumerated under
Section 1 of the Rules and Regulations Implementing the Provisions of PD No. 997 (Revised
2009) from a school recognized by the Commission on Higher Education at the time of filing of
application.
DEFINITION AND COVERAGE OF FIELDS OF STUDY FOR STSE

Pursuant to Section 1 of the Rules and Regulations Implementing the Provisions of PD No. 997
(Revised 2009), which is in accordance with Section 2 of PD No. 997, the term “Scientific and
Technological Specialist” shall refer to a person who has a highly technical qualifications gained
through education and sharpened by research and experience, and completed at least a bachelor’s
degree from a college, university, or institution recognized by the Commission on Higher
Education, with major or specialization in any of the following fields of study:

APPROPRIATENESS OF STSE

The STSE shall be considered appropriate for first and second level positions in the government
that do not involve practice of profession and are not covered by Bar/board/other special laws.

DATE OF EFFECTIVITY OF STSE

The date of effectivity of the STSE shall be the date of approval of an application by the PD No.
997 Committee.

PERIOD OF FILING OF APPLICATION FOR STSE

Filing of applications for the grant of STSE is continuous.

ASSESSMENT/EVALUATION OF APPLICATION FOR STSE

Applications are assessed/evaluated by the Department of Science and Technology (DOST),


through the PD 997 Committee, as to approval, or disapproval, for the grant of STSE.
Only approved applications are officially endorsed by the PD 997 Committee to the Civil Service
Commission (CSC) Regional Office concerned for the grant of STSE by way of processing and
issuance of the corresponding Certificate of Eligibility.

DOCUMENTARY REQUIREMENTS

 Upon Filing of Application at the Department of Science and Technology (DOST)

1. Duly accomplished CS Form 101-C, Revised Dec. 2011;

2. Three (3) pieces of identical I.D. pictures taken within the last three (3) months prior to filing
of application with specifications, as follows:

a. Passport size (4.5 cm x 3.5 cm)

b. Printed on quality photo paper;

c. In white background;

3. Assessment fee of PhP100.00 payable to DOST; and

4. Five (5) copies of each of the following documents:

a. Duly certified statement of duties and responsibilities;

b. Original and photocopy of transcript of records and diploma. The original copy shall be
returned to the applicant after authentication of the photocopy;

c. Recommendation from head of office or school on the application for PD 997 eligibility,
which shall include a statement regarding his/her assessment of the applicant’s research or
teaching proficiency;

d. List of S&T subjects taught/being taught and the duration of teaching said subjects, duly
certified by the Dean of the school; and

e. Other documents such as:

- Published research paper/technical reports for concluded scientific research, or progress reports
for researches still in progress. The reports must be certified as true copy by authorities to whom
the original copies were submitted;

- Certification of research proficiency indicating the title of scientific research project/s the
applicant has participated in and the duration and nature of participation and/or responsibilities
of the applicant in the research project; and

- Certificate of patented invention if available.

 Upon Claiming of Certificate of Eligibility at the Civil Service Commission (CSC) Regional Office

1. Original and photocopy of any of the following I.D. cards, which must be valid (not expired)
upon filing of application, and bears the applicant’s complete name, picture and signature, and
the issuing officer’s name and signature:

a. Current Office/Company I.D.
