Sean  Smith  is  a  kind  and  caring  boy  who  is  a  pleasure  to  teach!    He  is  a  very  
hard  worker  and  wants  to  please.    Sean  Smith  participates  appropriately  in  
class  and  always  follows  school  and  classroom  rules.    Sean  Smith  is  a  very  
motivated  learner  in  the  areas  of  math,  science  and  social  studies.    I  have  
observed  his  motivation  to  learn  drop  slightly  when  it  comes  to  reading,  
spelling  or  writing  tasks.    Socially,  he  has  made  several  friends  within  our  
class  and  engages  with  them  appropriately.    He  has  a  positive  attitude  
towards  school  and  is  a  role  model  for  other  students.  
 
Sean  Smith  is  very  strong  in  math!    He  was  able  to  quickly  fill  in  some  
kindergarten  skills  at  the  beginning  of  the  year  and  has  soared  to  a  new  
level  with  his  math  skills.    Sean  Smith  received  all  proficient  scores  on  his  
first  and  second  trimester  math  report  cards.    He  has  even  mastered  more  
difficult  skills,  like  counting  mixed  coins,  making  change  under  a  dollar,  and  
telling  time  to  the  5  minutes.      
 
In  math  this  year,  Sean  Smith  is  in  the  co-­‐teaching  math  class,  where  guided  
math  groups  are  often  utilized  to  practice  new  skills  and  re-­‐teach  previously  
taught  skills.    Sean  Smith  does  attend  math  intervention  for  30  minutes  
every  other  day.  
 
In  the  area  of  communication  arts,  Sean  Smith  participates  in  part  of  whole  
group  reading  instruction.    Sean  Smith  goes  to  two  different  reading  
intervention  groups.    One  group  focuses  on  phonics  and  the  other  focuses  
on  fluency  and  nonsense  words.    He  may  be  receiving  a  third  intervention  
for  sight  words.    Sean  Smith  also  works  with  me  for  15  minutes  daily  during  
guided  reading  groups.    
 
In  the  area  of  communication  arts,  segmenting  phonemes  and  printing  all  
upper  and  lowercase  letters  are  areas  of  strength  for  Sean  Smith.      
 
Phonics:  Sean  Smith  was  able  to  identify  23/23  consonant  letter  sounds,  
5/5  short  vowel  sounds  and  5/5  long  vowel  sounds.    When  using  decoding  
skills  to  read  CVC  words,  Sean  Smith  read  1/15  words  in  November  and  
7/15  words  in  March.    He  read  0/15  words  with  short  vowel  and  consonant  
blends  and  1/15  words  with  short  vowels  and  digraphs.    On  weekly  reading  
assessments,  Sean  Smith  answered  11/15  questions  correctly  during  the  
first  trimester  and  31/50  during  the  second  trimester.    When  reading  words  
that  follow  phonics  patterns  from  our  first  grade  core  reading  curriculum,  
Sean  Smith  read  30/45  words  correctly  during  the  first  trimester  and  21/45  
during  the  second  trimester.    During  the  third  trimester,  Sean  Smith  has  
answered  8/20  phonics  questions  correctly.  
 
High  Frequency  Words:    Sean  Smith  read  20/43  sight  words  and  answered  
11/15  questions  correctly  on  weekly  reading  assessments  during  the  first  
trimester.    During  the  second  trimester,  Sean  Smith  read  8/48  sight  words  
and  answered  21/50  questions  correctly  on  weekly  reading  assessments.    
During  the  third  trimester,  Sean  Smith  has  answered  7/20  high  frequency  
questions  correctly.  
 
Comprehension:    During  the  first  trimester,  Sean  Smith  answered  11/15  
comprehension  questions  on  weekly  reading  assessments.    He  answered  
21/50  during  the  second  trimester  and  10/20  so  far  during  the  third  
trimester.  
 
Based  upon  information  observations  during  whole  group  reading,  Sean  
Smith  listening  comprehension  skills  are  on-­‐grade  level.    He  is  able  to  
answer  literal  and  inferential  question  based  upon  the  weekly  reading  
story,  which  is  read  aloud  to  the  entire  class.  
 
Fluency:    When  asked  to  read  nonsense  words,  Sean  Smith  read  30  correct  
sounds  per  minute  and  8  whole  words  per  minute  at  the  end  of  the  first  
trimester.    The  benchmark  scores  are  35  correct  sounds  per  minute  and  5  
whole  words  read  per  minute.  
 
At  the  end  of  the  second  trimester  when  asked  to  read  nonsense  words,  
Sean  Smith  read  59  sounds  correct  per  minute  and  11  whole  words  pet  
minute.    The  benchmark  scores  are  51  sounds  and  11  whole  words.    When  
asked  to  read  a  first  grade  reading  passage,  Sean  Smith  read  10  words  
correct  with  67%  accuracy.    The  benchmark  score  is  28  words  correct  with  
81%  accuracy.  
Conventions:    During  the  first  trimester,  Sean  Smith  spelled  21/30  list  words  
correctly  and  0/6  high  frequency  words  correctly.    During  the  second  
trimester,  Sean  Smith  spelled  49/100  list  words  correctly  and  5/20  high  
frequency  words  correctly.    When  writing  words  that  have  not  been  
phonetically  taught,  Sean  Smith  struggles  to  phonetically  represent  all  of  
the  sounds.    For  example,  he  wrote  “bin”  for  bring.  
 
Beginning  in  the  third  trimester,  Sean  Smith  now  works  on  a  replacement  
list  of  spelling  words.    These  words  are  chosen  based  upon  the  phonics  
skills  he  is  currently  working  on  in  small  group.  
 
Overall,  Sean  Smith  struggles  with  decoding  words,  recognizing  sight  words,  
and  spelling.    Currently,  he  participates  in  whole  group  activities  where  the  
correct  spelling  or  reading  of  words  can  be  introduced  and  modeled  for  
him.    During  any  small  group,  partner,  or  independent  practice  activity  
related  to  reading,  spelling,  or  writing,  I  will  work  with  Sean  Smith  one-­‐on-­‐
one  or  in  a  small  group  with  modifications  and  adaptations  to  meet  his  
needs.  
 
For  example,  during  writing  activities,  Sean  Smith  will  orally  tell  me  the  
sentence  he  wants  to  write.    We  will  count  the  number  of  words  in  the  
sentence;  I  will  draw  a  line  for  each  word  and  write  the  beginning  letter  of  
each  word  on  the  lines.    I  will  also  complete  the  entire  words  for  words  I  
know  will  give  Sean  Smith  a  difficult  time  spelling.    He  will  fill  in  the  rest.  
 
During  phonics  practice,  the  activities  vary.    Most  of  the  time,  we  will  work  
on  a  short  vowel  phonics  skill  from  small  group  reading.    This  will  be  
different  from  what  the  rest  of  the  class  is  practicing.  
 
During  spelling  activities,  Sean  Smith  is  provided  with  his  own  replacement  
list  of  spelling  words.    For  the  most  part,  Sean  Smith  is  able  to  participate  in  
our  practice  activities,  just  with  a  different  set  of  spelling  words.    If  
everyone  else  is  rainbow  writing  their  words,  Sean  Smith  is  rainbow  writing
