ASSUMPTION OF RISK / RELEASE OF LIABILITY / COVENANT NOT TO SUE 

In consideration of being permitted to participate in any way in Ladies of Paradise’s Neon Safari event (hereinafter 
called the "Event"), I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, 
knowingly and voluntarily do hereby release, waive, discharge, and covenant not to sue Ladies of Paradise, LLC, its 
officers, employees, affiliates, members, staff, volunteers, representatives, and agents (herein after called “Ladies of 
Paradise”) from liability from any and all claims or causes of action including the negligence of Ladies of Paradise 
resulting in personal injury, accidents or illnesses (including death), property loss, and damages arising from, but not 
limited to, participation in the Event, including traveling to and/or from an event or location related to the Event. 
_X_I certify that I am 21 years of age or older and acknowledge that all participants must carry current, valid 
identification at all times while at the Event. 
*Non-US  Citizens  must  carry  valid passports at all times. Ladies of Paradise reserves the right to deny any participant 
entry  to  the  Event,  or  remove  any  participant  from  the  Event,  for  failure  to  carry  or  provide  proper  and  valid 
identification. 
_X_I certify that I have no physical or emotional problems, or history thereof, which would prevent me from 
participating in the Event, and that I have not been advised not to participate by a qualified medical professional. 
_X_I acknowledge that participation in the Event carries with it certain inherent risks that cannot be eliminated 
regardless of the care taken to avoid injuries, which may vary but range from: (1) minor injuries such as lack of 
hydration, scratches, bruises, and sprains (2) major injuries such as eye injury or loss of sight, joint or back injuries, 
heart attacks, vehicular traffic accidents, and concussions to (3) catastrophic injuries including paralysis and death. 
I HAVE READ THE FOREGOING PARAGRAPHS AND I KNOW, UNDERSTAND, AND APPRECIATE THAT 
THESE AND OTHER RISKS ARE INHERENT IN THE EVENT. I HEREBY ASSERT THAT MY 
PARTICIPATION IN THE EVENT IS VOLUNTARY AND THAT I KNOWINGLY ASSUME ALL SUCH RISKS. 
Marijuana Remains Illegal Under Federal Law. I understand that marijuana remains illegal under federal law. It is a 
Schedule-I controlled substance. Even in those jurisdictions in which the use of marijuana has been legalized at the 
state level, its use and consumption is a violation of federal law. The United States Supreme Court has ruled in 
Gonzalez v. Raich that the federal government has the right to regulate and criminalize marijuana. 
Removal of Participant from Event. I acknowledge that Ladies of Paradise reserves the right to remove me or any 
other participant from the Event for any reason. 
Indemnification and Hold Harmless: I agree to INDEMNIFY, DEFEND, AND HOLD Ladies of Paradise harmless 
from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees 
brought as a result of my involvement in the Event. If Ladies of Paradise incurs any of these types of expenses, I agree 
to reimburse Ladies of Paradise. I acknowledge that Ladies of Paradise is not responsible for errors, omissions, acts or 
failures to act of any party or entity conducting any event related to the Event on behalf of Ladies of Paradise. 
Medical Treatment Authorization: I understand that an emergency may develop which necessitates the administration 
of medical care. In the event of injury or illness, I authorize Ladies of Paradise to secure appropriate treatment 
including the administration of an anesthetic and surgery. I understand that such treatment shall be solely at my 
expense. Notwithstanding this paragraph, I understand and agree that Ladies of Paradise has no obligation to provide 
or seek out any medical treatment for me. I am aware and understand that I should carry my own health insurance. 
Severability: I expressly agree that the foregoing waiver and assumption of risks agreement is intended to be as broad 
and inclusive as is permitted by the law of the State of Oregon and that if any portion thereof is held invalid, it is 
agreed that the balance shall, notwithstanding, continue in full legal force and effect. 
Acknowledgement of Understanding: I have read this waiver of liability, assumption of risk, and indemnity 
agreement, fully understand its terms, and understand it cannot be modified orally. I covenant that this agreement was 
entered into at arm's-length, without duress or coercion, and is to be interpreted as an agreement between two parties 
of equal bargaining strength. I further understand that I am giving up substantial rights, including my right to sue. I 
acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and 
unconditional release of all liability to the  
greatest extent allowed by law.
