La adhesión de las bacterias gram negativas a. las células del epitelio uro-
telial ocurre gracias a las fimbrias de suu superficie, mientras que los microor-
ganismos grain positivos se adhieren más frecuentemente por p<)lisacáridos

Las E. Coli producen diferentes tipos de fimbrias con diferentes propieda-
des antigénicas y funcionales cmi la niisma célula, otras bacterias son capaces
de producir solo un tipo de fiinbria y en algunos aislamientos no es posible
detectar ninguna.
Las fimbrias están definidas funcionalmerute por su capacidad para me-
diar la hemaglutinación de tipos específicos de eritrocitos:

Durante la gestación hay dilatación de los ureteres y pelvis renal con mar-
cada disminución de la peristalsis ureteral, la vejiga también disminuye el
tono y aumenta su capacidad. Estos cambios comienzan bacía la séptima se-
mana de gestación y progresan hasta el parto, suelen ocurrir con mayor fre-
cluencia en el lado derecho y en el primer embarazo. En la patogenia de los
mismos se La sugerido un papel del hiperestrogenism&6-6 que explicaría la
aparición de éstos en mujeres que toman anticonceptivos orales>’.
La presencia de bacteriuria asintomática en al nltmjer gestaiíte es dei 4-7%.
La marcada dilatación de los ureteres duramite los últimos meses del embara-
zo, permite a las bacterias (le la vejiga alamzar el tracto urínaiio superior y
producir pielonefritis sintomática.
La mayoría de mujeres que <lesarrolian bacteritíria durante la gestación
tienen infección en la primer trimestre del embarazo. Sin embargo entre el 1-
1,5% del 25% dc las que sufren bacteriuria desarrollan infección en el último
trimenstreíiS.É». El 20-40% de pacientes con bactejiuria temprana en el emba-
razo desarrollan pielouíefrilis aguda sintomática tardía. En contraste ínenos
del 1% de pacientes con orina no infectada desarrollan pielonefritis. Por lo
tanto aproximadaunente el 75% de casos de pielonefritis aguda pueden preve-
nirse tratando la bacterimiria asintomática precozo.su.
l)IABETES MELLITUS
La bacteriuria es más frecuente en mujeres diabéticas que en aoíuellas que
no lo somi, debido> a una asociación entre los factores (le riesgo locales y del

huesped. Las infecciones del tracto urinario superior también son más fre-
cuentes en este grupo, incluyendo el absceso intrarrenal y otras complicacio-
nes poco frecuentes como la pielitis enfisematosa.
