TOP 10 INFRACTIONS:

1. Tardiness
2. Absences (AWOL)
3. Job abandonment (when the unauthorized and unexcused absence from work exceeds 5
consecutive working days with no advice from the employee).
4. Neglect of or non- compliance with standard operating procedures.
5. Failure to observe standard operating procedure tantamount to neglect.
6. Willful or intentional non-observance of standard operating procedure in the handling of any
transaction or work assignment for purposes of personal or another person’s advantage, profit
or gain.
7. For Management Team: Abuse of status, power, or discretion.
E.g. borrowing money from subordinates or asking them to guarantee for their loans.
8. Quitting work or leaving work area and assignment without permission from the immediate
superior before the official quitting time.
9. Prolonging/extending authorized break.
10. Gross or habitual neglect by the employee of his / her assigned duties.

Suggestions for COC process:

Tardiness – first offense has to be verbal warning by the Department Head, Second
Offense is written warning. (as per suggestion mostly by the Dept Heads)
 Absences – It must first pass through the Supervisor/Department Head of the employee,
Department Head may submit a recommendation or conference slip to the HRD.
 Other small matters e.g. lost receipt, lost documents, work errors and/or damage
equipments wherein no specific person is liable - It must pass through the Department
Head.
