TITLE QHSE HIRARC

1. Purpose

Managing the hazards/risks at workplace is vital in ensuring a safe and healthy working
environment. As such, any potential hazard and risk to people, equipment and material
shall be identified, their risks assessed and control measures developed for
implementation.

2. Scope:

This procedure is established to provide a pro-active action and guidance to achieve the
following objectives:

 Identification of hazards/risks at the work place;


 Assessment of the hazards/risks identified;
 Planning for the development of risk control;

3. Reference

OHSAS 18001 Clause 4.3.1Hazard Identification, Risk assessment and


determining controls.

4. Responsibility:

Person In Charge (PIC) for any work place of YFGB and other related subsidiaries are
responsible for the implementation of this procedure. PIC shall identify, assess the
hazards/risks, develop and implement the necessary control measures. The Site
Engineer, Site Supervisor and Safety and Health Personnel shall assist the PIC.

5. Procedures

5.1 In understanding the hazards/risks management at the work place, the following
situations should be considered:

 What can go wrong?


 How serious would it be?
 How likely is it to happen?

The following methods can be considered to facilitate the process of identifying


the hazards and assessing the risks at the workplace:

 Review of work activities for all occupations;


 Review of critical task analysis and procedure;
 Review of inspection report analysis;
 Review of accident/incident analysis;
 Review of task observation reports;
 Review of change to risk control measure;
 Review of operating procedures and method statement;
 Review of applicable regulations, codes and standards;
 Review of QHSE Management System audit findings;

 Review of material safety data sheet and Chemical Health Risk Assessment
(CHRA);
 Consultation/communication with workers;

5.2 At the work place, the Management Team shall determine as appropriate the
arrangement, mechanism and party that is to involve in the process.

5.3 Hazard Identification

5.3.1 In identifying the hazards for related activities, the list of common
hazards identified can be used as guidance.

5.3.2 For any construction project, the hazards for all activities that are carried
out or to be carried (as planned in the Construction Program) need to be
identified at the initial stage.

5.3.3 To facilitate the process of identifying the hazards, each job task for the
activity should be determined in order to identify all the possible
hazards. Other source of information e.g. the method statement may be
used in the process.

5.4 Risk Assessment

5.4.1 Risk should be determined using the following:

RISK = PROBABILITY x SEVERITY

Where:

• Severity: the potential injury/loss exposures to people,


equipment, material, and working environment.
• Frequency: how often it can potentially happen?
• Probability: what is the chance of the loss exposure to happen?

5.4.2 Once free hazards have been identified, the level or rates of risks need
to be classified. To determine the risk classification, the "Qualitative Risk
Table (Matrix)" as in QHSE-F41a and Severity / Likelihood Table QHSE-
F41b shall be referred.

5.4.3 The results of the Hazard Identification, Risk Assessment & Risk Control
Analysis shall be documented in (QHSE-F41).

5.5 Risk Control

Upon assessing the risks, control measures shall be developed while


considering the followings:

 Prioritization of hazards,
 Identification of affected groups;
 Employee participant;
 Legal requirements of control measures;

 Action plan
 Follow-up system
5.6 Control Measures

The control measures recommended shall be based on the hierarchy of control


(from most effective or at least effective or 'back up' control measure) as follow:

 Eliminated
 Substitute
 Isolate
 Engineering Control
 Administrative Control
 Personal Protective Equipment (PPE)

5.7 Chemical Health Risk Assessment (CHRA)

5.7.1 For any work place that use the hazardous chemicals as specified in the
schedules of 'Use the Standards of Exposure of chemicals Hazardous to
Health Regulations 2000' (USECHH 2000), the management team shall
make the necessary arrangements for the implementation and
compliance to the regulations.

5.7.2 The guidelines established for the CHRA shall be used as reference i.e.
for Chemical Register, methodology of assessment, assessor etc.

5.8 Safe Work Instruction (SWI)

5.8.1 SWI would stipulate all the necessary and recommended control
measures for particular activity in order to reduce or eliminate the risks.

5.8.2 SWI shall be established for activities with risks classified as 'HIGH' and
'MEDIUM' only.

5.8.3 The PIC of the work place shall make all the necessary arrangements for
the establishment of SWI.

5.9 Review

5.9.1 HIRARC process shall be reviewed at least once a year or


predetermined period (by the management) to evaluate the
effectiveness of control measures established in reducing or minimizing
the risks.

5.9.2 HIRARC (for relevant activity) shall also need to be reviewed whenever
the following events take place:

 New construction technology/activity/methodology


 New legal and other requirements
 Etc.
5.9.3 The results of HIRARC review shall be recorded.

5.10.1 Should there be any new activity, which is not in the List of Common
Hazard during the process of HIRARC, the QHSE Department at YFGB
shall be notified the purpose of updating the List of Common Hazard.
The Department shall then circulate the information on any new hazard
and control measures to all work places. The QHSE Department shall
then update the List of Common Hazard at least once a year.
