AFFIDAVIT OF THE SAME PERSON

I, Bernadeth Carvajal – Besonia, of legal age, married and a resident of Brgy.

Becuran, Sta. Rita, Pampanga depose and states:

1. That I was born on February 10, 1978 at Lubao, Pampanga, to the

spouses Pacifo C. Carvajal Jr. and Abremila S. Carvajal as evidenced by my
certificate of birth issued by the Philippine Statistics Authority or formerly known
as National Statistics Office. A copy of such is hereto attached as “Annex A” and
made integral part hereof;

2. However, as per my UNIFIED MULTI-PURPOSE (UMID)

ID which has the ID No. _______ issued here in the Republic of the Philippines, my
name was erroneously entered as BERNADETH CARUAJAL – BESONIA
respectively, contrary to my true and correct name which is BERNADETH
CARVAJAL – BESONIA respectively, as adverted above. A copy of the said
UMID ID is hereto attached as “Annex B” and made integral part hereof;

3. That such erroneous entry in my adverted UNIFIED MULTI-

PURPOSE (UMID) ID, specifically referring to my name was a mere product of
inadvertence and clerical mistakes;

4. That I already reported such matter to the government agency

concerned and the latter informed me that the correction of the said erroneous
entry is still being processed and being fixed in the present time;

5. That BERNADETH CARUAJAL – BESONIA born on February 10,

1978 at Lubao, Pampanga and BERNADETH CARVAJAL – BESONIA born on
February 10, 1978 at Lubao, Pampanga, refers to one and the same person;

6. That I am using the name BERNADETH CARVAJAL – BESONIA to

my other government issued identification cards such as my driver’s license issued
by the Land Transportation Office and my PRC membership ID issued by the
Philippine Regulatory Commission. Copies of these identification cards are hereto
attached as “Annex C and D” and made integral part hereof;

7. That I am executing this affidavit to attest to the truthfulness of the

foregoing statements and for whatever legal intents and purposes it may serve.
