PROJECT BACKGROUND/RATIONALE

The Provincial Government of Apayao and the Office of the Congresswoman in

coordination with Ay Foundation, Inc. will be conducting a Surgical/Medical Mission on
May 22-June 3, 2018 at the Far North Luzon General Hospital and Training Center,
Luna, Apayao.

In consonance with the thrust of the present leadership to provide affordable and
accessible health care services, thru effective coordination with AY Foundation, the said
medical/surgical mission will push through. It will benefit all Iapayaos especially those
who cannot afford to undergo surgical medication on particular cases like: Thyroid
mass, cleft lift/ palate, breast mass, ovarian cysts, cataract and many more.

One of the outreach programs of the AY Foundation is the conduct of said

Medical/Surgical Mission in remotest areas of the country in order to reach out and help
these constituents who cannot afford to go through medications in health service
providers. According to Ay Foundation, the province of Apayao is the second
beneficiary of the said program for this year in the country. It sees to reach out to three
more provinces and the province of Apayao is opportuned enough to be chosen as a
beneficiary of the noble charity program. Pool of surgical doctors and nurses will be part
of the team. Accommodations, transportation, medicines and
OBJECTIVES/PURPOSE

In general, the project aims to reach out to all Iapayao constituents who are in need of
medical attention specially those cases covered by the said medical/surgical mission.

EXPECTED OUTPUTS/DELIVERABLES

The following are the expected outputs/deliverables related to the project:

1. Orderly and a more strategic screening process of all interested patients;
2. Safe and responsible operations procedure to all eligible patients.

Van hire 30,000.00

The approval of this proposal is essential for the successful conduct of the
Surgical/Medical mission.
