completed before joining.

With regard’s from the HR DEPARTMENT INDUCTION TEAM , this is to inform you that
following documentation need to be proceeded and send in scanned format over the company
corporate mail id i.e. hrscopeservices@gmail.com, hr@scopeservices.org, by 10th of July 2012.

Since the operations team is finalizing the blue print of Pilot project operations and due the delay
in the finalization of pilot sites we are forced to shift the date to 15th of July 2012 for orientation
and joining. However as mentioned in service agreement it shall be admissible from 30th of June
2012. The venue of orientation will be finalized by operations team by 30th of June 2012 and you
shall be intimated over your mail.

You are requested to scan the following document from your side and send them by 10th of July
2012 to the corporate mail ids mentioned above.

1. ALL EDUCATIONAL CERTIFICATES.

2. PHOTOGRAPH OF SELF IN FORMAL POSE AND ALL THE FAMILY MEMBERS
WHOM YOU ARE DECLARING AS DEPENDENTS FOR CASHLESS MEDICAL
INSURANCE.
3. FULL SCANNED COPY OF SERVICE AGREEMENT AND PREMIUM DEPOSITED
WITH THE CORPORATE ACCOUNT.
4. KINDLY GIVE THE FOLLOWING INFORMATION FOR COMPLETING YOUR
MEDICAL AND INSURANCE POLICIES.
