Questionnaire

What are your core values? Why are those important to you?

This is important to the interview because understanding what the person values gives an

overview of the type of person they are. These are abstract, intangible concepts of importance or

meaning, such as time, health, honesty, and creativity. (Seaward, 2013) A person’s values are

what brings meaning to their life. Some examples are love, honesty, freedom, joy, wealth,

education, privacy, and creativity. (Seaward, 2013) Values typically reflect how a person thinks

or acts on a day to day basis.

Do you feel that your life is in tune to those core values? Why or why not?

This follow up question is important because although a person may value certain things, their

actions do not follow all the time. It gives the interviewee a chance to reflect on how important a

value is to them.

What makes you feel happy? Sad?

This question is also a follow up to the values question. It is just asking it in a simpler term.

Happy and sad are two easy to understand emotions so most people can relate to what makes

them feel either way. I ask happy and sad because it encourages a negative person to look at the

positives and a positive person to be honest about what makes them sad. These basic feelings

guide most of our actions. If it makes us happy we do more of that activity.

What does spirituality mean to you?

This question is important because understanding what their definition of spirituality needed to

know their perspective. There is a wide range of this definition. Many people use religious and

spiritual interchangeable, so asking this questions shows if they base their spirituality off church

only.

Do you believe in a higher power? Why or why not?

This question will give a look into their beliefs about believing in intangible power. Having a

connection to a higher power shows that they feel the world is bigger than just them. The

question is asked as higher power versus God because some people believe in other powers

besides God.

What daily activities do you do to promote your spirituality?

This question is important because asking about specific activities versus ideas show how well

the person actively cares about spiritual health. What a person spends money and time on shows

what is really important to them.

Would you describe yourself as spiritual? Why or why not?

This is an important question because understanding spiritual well-being helps show if the

person is on track to be a spiritual person. Spiritual well-being is described as the maturation of

higher consciousness through strong nurturing relationships with both self and others; the

development of a strong personal value system and a meaningful purpose in life. (Seaward,

2015)

How do you balance your spiritual well-being?

Understanding how they care for their spiritual health is important. All aspects of health are

important. In a healthy wellness paradigm, it is necessary to integrate, balance mental, physical,

emotional, and spiritual well-being. (Seaward, 2015)

Is daily mediation, prayer, or alone time important to you? Why or why not?

This question is important because meditation is important to many aspects of life. In Essential

Spirituality Rabbi Nachman states that " A person who does not meditate cannot have wisdom.

He may occasionally be able to concentrate...but his power of concentration remains weak and

cannot be maintained." (Walsh, 2013) The response can help determine how spiritually well the

person is. Even if they do not consider it meditating, having quiet time is beneficial.

What are your top 5 life stressors? How do you deal with life stressors?

Everyone has some types of stressors in life. How spiritual well a person is shows how they deal

with them. Having the interviewee list out specific examples will help them give a personalized

answer versus a general cliché answer. It will show how effective their muscles of the soul are.

Identifying the stressors are not negative if something is being done to improve them. Using

inner strengths like humor, compassion, patience, imagination, and creativity. (Seaward, 2015)

What is your golden rule of life?

After answering all the other questions, the person should have a better understanding of their

golden rule. This is a simplified statement of what the person values and how they want to live

their life. The basic golden rule most people are taught is to do into others, as you would like

done to yourself. However, a personal golden rule can be whatever is personally important to

them.
