SITUATED AT PUROK 4, SO. BANACOD, BRGY.

Please be informed that undersigned conducted necessary

inspection/investigation of the above-noted subject per request of Elisa Antonio Lim
dated February 20, 2017.

Per Findings;

1. That on February 22, 2017, 7:00 am, the undersigned proceeded to the area
at Purok 4, So. Banacod, Brgy. Canomay, Dimasalang.
2. Upon Arrival, the undersigned coordinated to the complainant, Ms. Elisa
Antonio Lim, she asked to the undersigned if there is a tree cutting permit
issued to certain Octavio Alim, Brian Afable, Marvin Mahinay and Efren
Gracia, then,
3. The undersigned found out that the one (1) Paho tree with diameter of 20 cm
x 3 mtrs, pruned at the perimeter boundary between Mr. Octavio Alim and
Elisa Antonio Lim and one (1) Anonang Tree and Paho tree were cut and only
stumps were found at the proposed barangay road.
4. The stumps of Anonang has diameter of 30 cm and Paho 20 cm respectively.
The area were portion of Lot No. 2520 Dimasalang Cadastre, Mangrove Area.
5. Thereafter, the undersigned, coordinated to Barangay Captain Pereus C.
Cabalan if there is Barangay Blotter report regarding the complaint of Ms.
Elisa Antonio Lim, he added that there is no brgy. blotter complaint report.

Recommendations;

The undersigned recommended that the offender Octavio Alim, et.al and the
complaint Ms. Elisa Antonio Lim be summoned to shadelight this matter that violates the
forestry laws and regulation of PD 705.

Sticker Posting at Brgy. Poblacion, Milagros, Masbate.

That on March 1, 2017, 8:30 am, the undersigned coordinated to the MPS,
PNP, Milagros, Masbate regarding the sticker posting “PAUNAWA SA LAHAT, Mahigpit
na pinagbabawal ang Pagputol, Pagbiyahe ng kahoy at iba pang produkto galing sa
kalikasan….”. The undersigned posted the sticker to some trimobiles and informed the
driver’s regarding Forestry Laws, Rules and Regulations of Presidential Decree 705 of
DENR. Especially the transport of lumber and wood Charcoals.

Please be informed that the Surveillance and Intelligence Unit Personnel,

NBI Agent Jimenez, and NISGSL conducted apprehension of 20 sacks of Wood
Charcoal situated at Brgy. Usab, Masbate City.

1. That on March 7, 2017, at around 4:45 pm, the undersigned conducted

Surveillance and Intelligence Operation at Brgy. Usab, Masbate City, while
the undersigned going back to Masbate City, one (1) unit tricycle load of
20 sacks wood charcoal flagged down the said trimobile and asked the
tricycle driver if there is a permit of legal document of transport of said
wood charcoal,
2. Likewise, the tricycle driver added he is only hired by unknown person to
transport the alleged illegal wood charcoal,
3. Thereafter, the undersigned apprehended the wood charcoal and declared
abandoned.

In compliance to relative of Memo of Regional Director Crisanta Marlene

P. Rodriguez received dated February 27, 2017, the undersigned conducted immediate
investigation regarding the occurrence of Grassfire situated at Brgy. Matiporon,
Milagros, Masbate, to wit;

FACTS: Conduct of immediate investigation on the information re:

Occurrence of Grassfire at Milagros, Masbate

FINDINGS:

1. That on March 7, 2017 at 2:30 pm, the undersigned proceeded to

Brgy. Matiporon, Milagros, Masbate,

2. Upon arrival at around 3:00 pm of same date, the undersigned

coordinated to Barangay Official on duty Brgy. Kagawad Magdalena
Godenis and Brgy. Kagawad Rudy Carmona regarding the information
sent through the DENR V-Facebook page on February 11, 2017,
relative occurrence to the Grassfire along hillside of Cattle ranchers,

INVESTIGATION:
3. Upon investigation to the on duty Kagawad, the undersigned found out
that the occurrence of Grassfire was originated from the pasture area
of certain Juvencio Revil adjacent along National Highway western
Hillside portion of Brgy. Matiporon, Milagros, Masbate,
 4. Moreover, the Grassfire occurrence was due to people passerby
cigarette bult that cause ignition of grassfire that mount the western
hillside of the pasture area of certain Juvencio Revil,

5. The pasture area that razed by Grassfire was approximately 5.0 has
and found to be within, Block-R T.L Project 4-B certified on February
14, 1925.

RECOMMENDATION:

The forest lands which area being utilized for pasture shall be maintained
with sufficient grass cover to protect soil, water and other forest resources. Hereof, and
the pasture lease must be construct fire break activity especially the onset of summer
session.

Please be informed that the undersigned conducted surveillance on the

reported illegal shipment of lumber on the vicinity of Balud, Masbate. The following
information were gathered during the surveillance, patrolling, to wit;

1. That on May 23-26, 2017, the undersigned coordinated to the Brgy.

Official regarding the alleged illegal shipment of lumber.

2. After ocular inspection and surveillance in the reported docked area, no

illegal shipment found and visible in the area.

RECOMMENDATION:

The undersigned recommends the regular surveillance and patrolling to

minimize the illegal shipment/ activities of forest products.

Please be informed that the undersigned conducted monitoring/ inspection

of the One Hundred One (101) pcs. Lauan Lumber apprehended/custody at PNP Aroroy
Station, Aroroy, Masbate.

The following information were gathered to wit:

1. Based on Police Blotter Report, on August 28, 2016 at around 7:50 am,
the elements of PNP Unit of PNP Aroroy Station apprehended (101) pcs.
Lauan Lumber with dimension of 2 x 6 x 20 with a total volume of (2,020)
bd.ft. at So. Cambatang, Brgy. Jaboyo-an, Aroroy, Masbate.

2. The said Lauan Lumber were abandoned/stocked beside the residential

house of Joel Molines Y. Lejo, 54 y/o resident at So. Cambatang, Jaboyo-
an, Aroroy, Masbate. (Pls see attached Police blotter Report).

3. That, after a year certain, Danilo L. Mendenilla, executed an affidavit

claiming the apprehended 101 pcs. Lauan Lumber (Pls. see attached
Affidavit of Mr. Danilo P. Corpus).

4. In view hereof, the undersigned recommends the presence of Mr. Danilo

P. Curpos to the next ACP hearing for proper collaboration of his Affidavit.
