PETITION

COMES NOW Petitioner unto this Honorable Court most

respectfully states that:
1. He is a Filipino citizen, of legal age, born on March 18, 1993,
presently residing at _____________________, and with office
address at Tambago and Diaz Law Office, Unit 2-503 OITC 2
Building, Oakridge Business Park, 880 A.S. Fortuna St., Banilad,
Mandaue City, Philippines, with Telephone No.: (032) 345-4748;
2. He is of good moral character and a member of the
Philippine Bar in good standing under Roll of Attorney No. 70590,
having passed the bar examinations given in the year 2017 and
admitted to the practice of law on June 4, 2018. A copy of the
Certifications issued by the Office of the Bar Confidant, Certification
issued by the Integrated Bar of the Philippines, and Certification
issued by the Regional Trial Court Criminal Cases Division, are hereto
attached as Annexes “A”, “B”, “C” and “D”, respectively, and likewise
made integral parts hereof;
3. He has never been convicted of any crime involving moral
turpitude;
4. He has never applied for notarial commission in any other
court of the Philippines;
5. He has attached three (3) passport-sized colored
photographs with light background taken within thirty (30) days of
this application with the latter’s signature at the bottom of the
photograph, marked hereto as Annex “E” and made an integral part
hereof;
6. Attached in a separate sheet of paper forming part of his
petition are three (3) specimens of Petitioner’s official signature,
marked as Annex “F”;
7. He has paid his IBP dues for the year 2018 with IBP OR No.
044148 dated June 5, 2018 and has paid his professional tax under
PTR No. 4499459 dated January 20, 2012. Copy of the Official
Receipt from the Integrated Bar of the Philippines and the 2011 PTR
are hereto attached as Annexes “G” and “H”, respectively;
8. He possesses all the qualifications and none of the
disqualifications for appointment as Notary Public.

PRAYER

WHEREFORE, premises considered, it is most respectfully

prayed of this Honorable Court that petitioner, LEO JOSELITO E.
BONO be granted a NOTARIAL COMMISSION for the City of
Mandaue and the Municipalities of Consolacion, Liloan and
Compostela and accordingly be issued a Certificate of Authorization
to Purchase a Notarial Seal.
Other reliefs as this Honorable Court may deem just and
equitable are likewise prayed for.

resident of Nasipit, Talamban, Cebu City, after being duly sworn in
accordance with law, hereby depose and state that:
 I am the Petitioner for the application of a commission as
Notary Public; I have caused the preparation and filing of this Petition
and have read and understood the contents thereof, and all the
allegations contained therein are true and correct based on my own
personal knowledge and information and authentic records;

I have not commenced any other action or proceeding involving

the same issues in the Supreme Court, the Court of Appeals, or any
other tribunal or agency; to the best of my knowledge and belief, no
such action or proceeding is pending in the Supreme Court, the Court
of Appeals, or any other tribunal or agency; if I should thereafter
learn that a similar action or proceeding has been filed or is pending
before the Supreme Court, the Court of Appeals, or any other
tribunal or agency, I undertake to report that fact within five (5) days
therefrom to this Honorable Court.
