Eligibility

 To be eligible to become a Varsity Player, an applicant must be a bona fide student of St. Mary’s
College of Tagum; not more than 24 years of age; of good moral character; physically and
mentally fit to play.
 He/she must be enrolled in a minimum of 16 units per semester with no dropped subjects and
failing marks in any subject. He/she must be able to fit in his /her schedule practices and
trainings.
 To maintain the scholarship, he/she has a general weighted average grade of at least 80% per
semester.
 The student must apply for the Mother Ignacia Varsity Sports Scholarship every semester and
pass the medical exam (including a drug test0, screening, qualifying/try-outs and interview by
the College Sports Coordinator and the Scholarship Committee.
 The student shall be required to sign a Mother Ignacia Varsity Sports Contract as drafted by the
Sports Coordinator and/or Director of Student Affairs and Services.
 It is also important that the student had never been subjected to any disciplinary action while
studying in SMCTI. (Please see Article VI on Student Discipline in the College Student
Handbook).

Requirements

 Copy of Mother Ignacia Varsity Sports Registration Form

 Copy of Senior High School Report Card( for freshmen) and Student Permanent Record( for
transferees and Old Students)
 Five (5) 2X2 I.D pictures with white background
 Medical Certificate (duly certified by the School Physician) with medical results:
CBC, Urinalysis, X-ray, Drug test
 BIR Certificate of Tax Exemption or Income Tax Return
 Accomplished Mother Ignacia Varsity Sports Contract
 Passed the screening committee, qualifying try-outs and interview
Application

 Application period: April 1 to May 31 ( dates are subject to change as a discretion of the HED
Sports Coordinator)
 Deadline of submission to the College Sports Coordinator: May 31 ( dates are subject to change
as a discretion of the HED Sports Coordinator)
 Forms are available at the Office of the College Sports Coordinator.
 Applications, together with all relevant supporting documents, must be submitted to the Office
of the College Sports not later than 5pm.

Attendance

 All Marian Hawks Varsity students are required to practice on Monday, Wednesday, Friday, and
Saturday at 5:00 am to 8:00 am regularly and punctually. It is also the discretion of the coach to
have a practice at 5:00 pm and the players have 15 minutes allowance to prepare themselves
before the practice.
 A letter of excuse from parent, guardian duly noted by Coach is required before a player is
admitted to practice.
 If a player comes 10 minutes before the practice, he/she is marked late but allowed to enter the
practice.
 Before a player is removed from the team, he/she is allowed 3 absences.
 Player is exempted due to following reasons: (1) School representative (2) Serious illness, (3)
death of any immediate member of the family, and (4) victim of calamities, and (5) in case of an
ill immediate family member.
