APPLICATION FOR SEARCH WARRANT

COMES NOW, the undersigned, POLICE SENIOR INSPECTOR RICARDO

DALISAY, Chief of Police of Malvar MPS, Malvar, Bulacan, after having been duly sworn
to in accordance with law do hereby depose and state the following under oath:

That he was informed and verily believes that JULIO DIAZ y PADILAA, who may
be found at the premises in Brgy. San Simon, Malvar, Bulacan, are in possession or has
in his control one (1) unit M16 (Armalite) rifle, one (1) unit pistol of unknown caliber, and
one (1) piece hand grenade which he is keeping and concealing in the premises above
described.

That the undersigned has verified the report and found it to be a fact and was
confirmed to him by his witness, ALYANNA DALISAY, who is able to gain entry several
times in the aforementioned premises of the respondent and has therefore reason to
believe that search warrant should be issued to enable the undersigned to take
possession and bring to this Court the following described property:

a. One (1) unit M16 (Armalite) rifle

b. One (1) unit pistol of unknown caliber

c. One (1) piece hand grenade

d. Dynamite and other materials used in making the same.

That the undersigned has verified the report and found out to be a fact after careful
perusal of records in their office yielded that JULIO DIAZA y PADILAA is NOT on the lists
of Gun Holders with firearms license and persons authorized to acquire and possessed
explosive and other incendiary device. (A Copy of List of Persons with Firearms License
hereto attached)

That after confirmation on JULIO DIAZ y PADILAA has no license to own and
possess firearms and explosives, suspect is therefore violating Section 28 (b) of Republic
Act 10591 “Comprehensive Firearms and Ammunition Regulation Act” and Section 3 of
Presidential Decree 1866 as amended by Section 1 of Republic Act 9516.

That the undersigned has believed that a Search Warrant be issued to enable the
undersigned to arrest the suspect, take custody of all his possession and to bring before
this Honorable Court the above-described pieces of evidence.
 WHEREFORE, the undersigned prays to this Honorable Court to issue a search
warrant authorizing him and his men or any peace officer to search the premises and if
machines are attached to the ground, padlock the premises described in this application
and to seize and bring to this Honorable Court the personal property above-described to
be dealt with the full accord with existing laws.
