FILM FOOTAGE RELEASE

This Agreement is with reference to use of the “______________” video footage

(“Clip”) of supplied and owned by _________LLC, a California Limited Liability
Company, (“Producer”) with its principal offices located at _______________, California
90265 and licensed by Universal Studios with its principal offices located in Universal
City, California.

In consideration of _____________($ ) for up to five minutes of said Clip used

in _____________, (the “Production”) and for other good and valuable consideration, the
receipt and sufficiency of which are hereby acknowledged, Producer grants to Universal
Studios, its successors, assigns and licensees, the non-exclusive and irrevocable right and
license to use said Clip in such manner as you may determine in your sole and subjective
discretion in the Production, and to exhibit, publicize, and otherwise utilize and exploit
the Production containing the Clip (and to use the Clip as contained in the Picture in
trailers and advertising in connection with the Production), in perpetuity and throughout
the universe, by any and all means and methods whether now known or hereafter created
or devised, including, but not limited to, film and/or video of all widths and gauges,
tapes, cassettes and discs, and in any and all media, whether now known or hereafter
created or devised, including, but not limited to, theatrically, non-theatrically, by all forms
of television, including but not limited to, free, pay, toll, subscription, cable and satellite
television, television devices, audio visual cassettes, cartridges and discs and to exploit
all so-called ancillary and subsidiary rights in the Production containing the Clip.

Nothing contained herein obligates you to exercise any of the rights, licenses or
privileges granted to you by this Agreement. This Agreement shall be construed in
accordance with the laws of the State of California applicable to agreements that are
executed and fully performed within the State. This Agreement contains the entire
understanding of the parties relating to the subject matter, and this Agreement cannot be
changed, altered or modified except by written Agreement executed by both parties
hereto.
