Defect Reference: Mail from BEML No-QHR/4580103453 DTD27.01.2018.

Material : Structure Description:- Beam, Column, bracing. Qty.:- 3 NOS

Defect description:
1)m20x2.5 tapped hole filled with grease which results in less visibility and more
Cleaning time. You are requested to use threaded plastic cap to protect thread from
Damage.
2) integral test piece have to be send along with consignment to check chemical
Composition at our end.
3) Rest preventive oil is not going with cleaning agent. Please inform us suitable
Cleaning agent or please use some alternate rust preventive oil.
4)part number embossing is fouling with assembly, to be shifted to the location shown in
Phote-1 as per drawing.
Correction: 1) In future supply we shall use plastic cap as advised.
2) Point noted, in future supply we shall give one integral test piece per heat.
3) In future we shall use only one coat rust preventive oil.
4) Point noted, we shall provide part number as per photo -1 only.
Root Cause: : 1) our general practice was to supply items with grease in threaded area .
2) Integral test piece was cut at our shop for testing.
3) We have applied two to three coat of rust preventive oil due to long storage time in
your open yard. Or kindly suggest the party name of rust preventive oil manufacturer.
4) Size of letter used for part number embossing was big.

Corrective Action: 1) Plastic cap of M20X2.5mm purchased and shall be used to protect
threads.
2) Instruction given to casting manufacturer to pour two no’s integral test piece per heat.
3) Alternate rust oil purchased which is easy to remove.
4) Instruction given to casting manufacturer to shift part number as per photo-1.

Dear Sir,

As per email from Mr. Pragnesh Tandel, following point no. 2 is still pending in CAPA.

2) Correction was not mentioned in report.

Please note that correction was done by WWIL Mechanical Assembly Technicians by enlarging the hole
in mating part (Brake Disc).

Also, please send CAPA in new format sent by me yesterday, forwarding the same again.

As per your defect Report No.-13, SL. No. of Disc rotor had not been specified so we have kept Sr.No. column empty.
