Re : Proposal for Health Screening For All SASB Staffs for 2018

Dear Director,

Regarding to the above matter, HSE Dept would like to propose Health Screening for all SASB staffs to be
conducted this year.

It has been brought to HSE Dept attention on occasions such as toolbox meeting and recent Safety
Committee meeting that SASB staffs has made inquiries on when will the next Health Screening will be
conducted.

It has also been pointed out by Supervisors from Production Dept that some of our workers fitness and
health are at a worrisome level.

Previous Health Screening was conducted in 2014 and 3 years have passed. It is our believed that Health
Screening should be conducted this year for the reasons stated below:

1. To know SASB staffs current health and fitness level

2. To detect changes in staffs health that may be related to harmful working conditions
3. Concerning number of production staffs with high blood pressure
4. To take preventive action by getting early treatment to avoid any incident during working

Attached with this proposal is the quotation for packages available and total cost estimate.

We hope you will take this proposal under consideration.
