A Failure Reporting, Analysis, and Corrective Action System (FRACAS) provides a process by which failures

can be reported in a timely manner and analyzed so that a corrective action system can be put in place and
eliminate or mitigate the recurrence of a failure. The goal of FRACAS is to help you better understand failures and
the reporting required to eliminate or control the failures that are killing reliability in most organizations.

FRACAS enables a Continuous Improvement Process applied to eliminate/mitigate asset failures with
modifications to current Equipment Maintenance Plans (EMP), Equipment Redesign, or Time Based
Replacement resulting in Optimizing Asset Health resulting in reduced cost and increased output.
