Sub: the original ATM Card having lost, request for a fresh\ duplicate one; Reg:

Respected sir,

I , N.K.Ayengar,with A\C No- 00000011349723488 operated at SBI Branch-Code - 0038,

Bhanjanagar, Ganjam, Orissa lost my ATM(debit) card (No: 6220180003800036752) on 21st
Jan 2010, and immediately got it blocked via T No: 201001214812406 dt 21.01.10 contacting the
Customer Care, Bhubaneswar, Orissa.
On 27th, Jan. 2010 I informed the BM, SBI, (concerned) about the mishap, with due request for
issuing me a fresh\ duplicate card. But with no response. So, once again I applied for the same
with a filled in form, a covering letter with a Xerox copy of the previous letter, in the way of
reminding him, in the middle of March, but to no effect. I had requested him, vide this letter, to
procure the ATM card and dispatch the Intimation Letter to my Hyderabad address (where I have
been staying since after my retirement in 2006).In that case I would go and collect the same in
person, I had thought. I just don’t know if there is any technical problem even as all this service is
done at the account holder’s expenses. In short, this account was my salary receiving account
during my Govt. service in Orissa and after my retirement it continues in the same manner in
receiving my monthly pension. Since there is no provision for inter – state transfer of pension
accounts, I can’t cancel this account in Orissa and depend solely on my ATM debit card for
withdrawal purposes in Hyderabad where I stay with my family.
The loss of the ATM card has dragged me to such a level below that I have to depend on my
children for small amounts of money (to buy medicines etc.) while my account in SBI branch
above has sufficient money unwithdrawn for want of the Card.
Could you, sir, help me in this regard. I request you to see that the ATM card is despatched to the
SBI branch concerned and the intimation letter to my present address. So that I could go in person
and collect the card. Or else, the BM, Bhanjanagar, after receiving the Card, may kindly be
directed to dispatch\ redirect the intimation letter to my present address which is as follows:
