 Secure application form from the
Admission Office and fill-up.
 Pay the P200.00 entrance examination
fee at the Cashier’s Office
 Submit the accomplished application
form for evaluation and intake
interview with the following
requirements:
- 2 pcs. 2 x 2 colored ID
picture
- Photocopy of report card
- Photocopy of NCAE result
- 2 pcs. long brown envelope
 Get the entrance examination permit.
