Policy
Exams will be accepted for regrade only if they are accompanied by a signed Regrade Request Form
(this form) and are submitted within one week after the exams are returned to students. When an
exam is submitted for regrade, the entire exam is subject to regrading.

Statement of Honesty
I am requesting to have my exam in Finance 251F (Foundations of Corporate Finance), with
Professor Simon Gervais, regraded.
I understand that I may not, under any circumstances, make any marks, changes, corrections,
explanations, clarifications, notes, and so forth on my original exam which I am submitting for a
regrade.
I am aware that my original exam may have been photocopied, and that therefore any alterations
found on the exam I submit for a regrade will be presumed to have been made after the exam was
originally graded and returned to me.
I understand that any alterations that do appear will be construed as a violation of the Honor
Code.
I affirm by signing this statement that I have read this statement, I understand its significance,
and that I have complied with its requirements.
