REQUIREMENTS FOR CHURCH WEDDING (THREE (3) MONTHS BEFORE THE WEDDING DATE)
CANONICAL INTERVIEW - schedule 1-2 months before wedding with church coordinator/parish priest you will be marrying in
BAPTISMAL CERTIFICATE - with annotation “for marriage purposes only” (acquired 3 months before wedding)
CONFIRMATION CERTIFICATE - with annotation “for marriage purposes only” (acquired 3 months before wedding)
MARRIAGE LICENSE - must be within 3 months validity period
CERTIFICATE OF NO MARRIAGE - from the National Statistics Office
MARRIAGE BANNS AND PERMISSION - Couples must know specific name of parish priest and the address of parochial church then submit to the church y

**Application form and 1 valid ID
BAPTISMAL CERTIFICATE issued within 6 months of wedding date and with notation "FOR MARRIAGE PURPOSES"

BAPTISMAL & CONFIRMATION CERTIFICATE issued within 6 months of wedding date and with notation "FOR MARRIAGE PURPOSES"
**Photocopy of Baptismal Certificate
INTERVIEW - the couple is required to undergo CANONICAL INTERVIEW WITH THE PARISH PRIEST 3 MONTHS BEFORE THE WEDDING
Every Wednesday at 8:00 in the morning at least 3 months before the wedding
**Questionnaires given by San Antonio De Padua Parish
PRE-CANA SEMINAR - the couple should attend a seminar every 2ND SATURDAY OF THE MONTH 8:00AM-12:00NN (once only).
Couple are allowed to attend the seminar in other parish, Discovery Weekend or Catholic Engaged Encounter are also accepted,
provided a certificate of attendance must be submitted.

MARRIAGE BANNS - the banns should be posted for 3 consecutive Sundays at their respective churches.
**Letter from SADP will be provided address to grooms' and brides' church respectively
MARRIAGE LICENSE - should be requested from the municipal office where the Groom/Bride resides. 120 days validity
>> Certified true copy of you and your partner's NSO Birth Certificates (1 original and 2 photocopies each)
>> Certificate of No Marriange (1 original, 1 photocopy)
>> Certificate of Attendance in a pre-marriage counseling (usually conducted by church)
>> Community Tax Certificate of Cedula (1 original, 2 photocopies)
>> Barangay Clearance (1 original, 1 photocopy)
>> 2 Valid IDs
>> Recent 1x1 photo
>> Marriage License Application Form (local registry office)
LIST OF SPONSORS - Submit a copy of the complete list of names of sponsors a month before the wedding date.
**Sponsors 1-6 will be written on the form issued by SADP, P/S in excess of 6 will be printed in a separate paper provided by couples
CONFESSION - one week before the marriage.
Anywhere the wish to have confession is allowed provided the certification of confession must be signed by the priest.
**Certificate issued from church where confession was made will be presented to SADP
REVIEW OF MARRIAGE CONTRACT - A DAY BEFORE THE WEDDING DATE, the couple should review the information on the
marriange contract before the final printing.
