of this letter, Mr. Mario A. Ramirez my father to claim my Transcript of Records (TOR), Good Moral and
Honorable Dismissal in your office. I am also allowing him to request and claim for a copy of the
authenticated TOR.

I won’t be able to apply and claim it personally because of an important reason that is why I have asked
him to bring this letter together with my identification card as valid proof. I am also giving him the right
to sign any papers that has connection with the application and claiming of the said documents.
