INVESTMENT AGREEMENT and PROMISSORY NOTE

For value received, I Mr. Jose Wilfrido S. Iluminado, of legal age, with residence and office address at
_______________________ Mabalacat City, Pampanga, referred as the “Borrower”, this 21st day of August
2016, do certify the following:
That I am indebted to Ms. ____________________, the principal amount of One Hundred Thousand
Pesos (P 100,000.00) subject to monthly interest rate of seven (5%) percent per month, which I
promise to pay in these terms;

1. September 21, 2016-P5,000.00 4. December 21,2016- P5,000.00

2. October 21, 2016 – P5,000.00 5. January 21,2017 – P5,000.00
3. November 21, 2016-P5,000.00 6. February 21,2017- P105,000.00

The Borrower will use the fund to finance his business/ businesses.
The Borrower will at all times take or cause to be taken all such action as may from time to time be
necessary to maintain, preserve and renew its existence as a business entity in good standing under the laws
of the Republic of the Philippines.
The Borrower shall have the right at any time and from time to time to prepay the outstanding
principal balance of this note, in whole or in part, without premium or penalty, provided that each such
prepayment shall include all interest then accrued but unpaid on the note.
If the Borrower defaults in the performance or observance of any of the conditions or agreements set
forth in this note, and if such default continues for more than 3 days, the Creditor may declare that the entire
balance of unpaid principal will be due immediately together with the accrued interest;
If any action or proceeding, including arbitration, is commenced to collect this note or any part thereof
at law, the Borrower hereby agrees to pay the collection charges, should collection be referred to a collection
agency or to payee’s collection facilities;
In the event of the Borrower’s failure to pay any of foregoing installments, the whole sum remaining
then unpaid shall immediately become due and payable, with additional ten (10%) percent interest at the option
of the holder of this note, and the Borrower further promise to pay to the holder of this note and additional sum
equivalent to 20% of the amount due, for and as attorney’s fees and expenses of collection, in addition to the
cost of suit.
