S.M.P. : I am knowledgeable of the interrelation practice between the client, the nurse, and the environment. I know that the complexity, the

Predictability, and the Risk of negative outcomes of the client determines whether an RN or an RPN cares for that client. I am mindful that the more

complex the client situation is and the more dynamic the environment should be (College of Nurses in Ontario, 2014 ).

S.M.P. : This approach is used throughout my practice, before performing a procedure I ensure I do my research to ensure my client's safety an

example of this is when administering medications I ensure I know all the side effects and contraindications of the drugs in to prevent harm to my

client.

S.M.P. : When caring for my clients I ensure I know their code status, allergies, mobility level and risk for fall. I ensure their environment is safe and

when assisting with a movement I use their assistive device in place. I promote a safe environment by ensuring the floor is free from clutters, the bed

is in the lowest position and call bell is put into the clients reach.

When I am uncertain about an order or plan of care I check the client's chart for clarification, I speak to my clinical professor and also the primary

nurse in charge of my client in order to provide the best care for my client safely.

S.M.P. : During my clinical placement, I have sought out guidance from my clinical professor when faced with an unsafe or unprofessional behaviour.

An example of this is in my client's file there is documentation to check and dress wound on the coccyx, however, my client states he no longer has a

dressing there, this was reported to my professor and then the nurse in charge of my client's care. After careful examination, it was brought to our

attention that the dressing was removed the previous day but the nurse did not document this action.

S.M.P. : The role of a nurse is to"promote, maintain and restore holistic health for individuals and communities (Association of Rheumatology Health

Professionals, 2002). This skill is achieved because as a student nurse I aid in the promotion, maintenance and the restoration of my client's health.

S.M.P. : I am an active participant in team sharing and discussions. An example of this is I participate in post-conference discussions, if given to

perform a new procedure I share my experience with my colleagues. I also presented to my clinical group about various topics such as stages of

amputation, Care of Stump, Stump conditioning, etc. If given to perform a new procedure I share my experience with my colleagues.

S.M.P. : Documentation can be defined as "anything written or electronically generated that describes the status of a patient or the care or service

given to that patient" (Potter & Perry, 2014 ). I document appropriately using written SOAP notes under supervision from my clinical professor I

believe this skill has been improved considerable throughout my clinical placement.

S.M.P. : When caring for my client if any situation arises I relate this information to my professor. An example of this is a client has medication order

for 2 drugs ( eye drops) at different times, she has been self-administering these meds wrong by using the one for 1000 at 1200 and the one for 1200

at 1000. After finding this I seek assistance from my clinical professor. The matter was then brought to the nurse's attention.

S.M.P. : I believe that using communication techniques with the client and the inter-professional health care team is one of my strengths. When

communicating with my clients I get down to there level, use language they understand, use therapeutic touch and in a case with a client that had a

language barrier I used sign hoping she would understand, I also used one of my peer who spoke Cantonese to translate.

S.M.P. : I ensure my client's privacy and confidentiality in the following ways I do not release any information about my client's health conditions, I

don't discuss details about my client's health with anyone who is not providing care for my client, etc.

S.M.P. : In my clinical placement, I use technology to access information including research, evidence-based data, and other health information an

example of this is I use my e-textbooks to provide me with information about the medications I am not knowledgeable about before I administer

medications. I also completed online e-modules for my facility, I access blackboard and I complete my knowledge for you evaluations and these all

include using technology.

S.M.P. : As mentioned above I believe that using communication techniques with the client and the inter-professional health care team is one of my

S.M.P. : This skill is accomplished when I advocate for my client and by being empathetic towards my clients.

An example of this can be seen as I attended an in-service on how to assist clients with amputee and mobility issue safely by using their assistive

device. By learning how to do transfers safely and using devices such as the sliding board I can now care for my client safely and foster their well-

being.

S.M.P. : I collaborate with clients and members of the inter-professional health care team to provide the best care that I can to my clients this can be

seen as communicating with my client's physiotherapist to schedule his therapy, reporting if any situation arises such as abnormal vital signs with the

charge nurse and collaborating with the occupational therapist to educate my self on how to transfer patients safely.

be showered one hour after breakfast instead of performing this task to better suit my schedule I used the opportunity to assist my colleagues with

their clients and then attended to my client giving her the time she needed,

S.M.P. : I am mindful that teaching a client to administer their medication correctly, providing information about a new diagnosis, etc are all ways of

providing client education, however, the opportunity to provide client education and evaluate its effectiveness has been limited throughout my

placement.

S.M.P. : I was not presented with a conflict during my clinical placement. I believe if faced with a conflict it must be confronted so as to create a

resolution so if I was presented with a conflict I would voice my concern to my professor and the person in charge of the unit, by doing this I would be

going through the line of command.

S.M.P. : An example of me possessing this skill is my peer had an incident at clinical that left her traumatized the following week of clinical she was

still shaken up so to alleviate the stress and to create a positive environment I worked together with her throughout the day and supported her.

S.M.P. : I Provide feedback to peers and accept feedback from peers. Feedback provided to me from my peers and professor is incorporated in my

S.M.P. : An example of me advocating for my client is my client states she hates eating the same food over and over and this had been going on for

the past 5 days so I suggested to the nurse in charge of her care that the dietitian should have a meeting with the client in order to plan meals that are

both healthy and her includes a variety.

Another example of me advocating for my clients when pain is reported based on severity I will suggest to the charge nurse that if the client has a prn

order I think it should be administered.

S.M.P. : When caring for my clients I consider their choice this can be seen demonstrated by asking if they want their shower a particular time, or with

a lotion they prefer or even asking what they prefer me calling them.

An example of me supporting my clients’ rights for self-determination and choice is client states she hates eating the same food more than one so I

suggested to the nurse in charge of her care that the dietitian should have a meeting with the client in order to plan meals that are both healthy and

her includes her preference.

S.M.P. : If presented with unsafe, unacceptable, and unprofessional behaviors as stated above I would voice my concern to my professor and the

person in charge of the unit, by doing this I would be going through the line of command. By confronting the conflict a resolution can be met.

S.M.P. : This was demonstrated by ensuring I received all necessary immunization before going into placement. This protected both myself and my

I provide patient center care are being responsible, being accountable in my practice, building a therapeutic relationship with my clients, using

S.M.P. : An example of how this skill is demonstrated in my practice is when assigned to a client with risk for falls I ensure I assess my client before I

start the transfer example are they experiencing orthostatic hypotension, if they have proper footwear on, if client is oriented and then I ensure that all

intervention is implemented example all wheels on wheelchair has the break on, bed in lowest position, etc

S.M.P. : I collaborate with the healthcare team if there is any change in my client's condition or expected outcome. An example of this is when taken

vital signs if any of the reading is abnormal then this is reported and based on severity medication may be administered, this shows that the changes

in the client's needs are met.

S.M.P. : Whenever given a skill that I believe I need development on I ask my professor or my nurse to supervise my procedure and incorporate

feedback for my future practice. An example of this is asking for supervision when applying a dressing to a wound that is complex or when

administering insulin to a diabetic client.

S.M.P. : I believe that with teamwork my client can get the best care that is required, however, in my placement I have not had a conflict with any

team member. As mention above If presented with unsafe, unacceptable, and unprofessional behaviors I would voice my concern to my professor and

the person in charge of the unit, by doing this I would be going through the line of command. This will confront the conflict and a resolution can be

met.

environment.

S.M.P. : A few strategies that I use to meet my client's needs in a changing environment are time management skills and teamwork skills. By using

time management skills I can ensure all my client's needs are met in an appropriate amount of time so as to ensure adequate time if any unexpected

changes occur in the environment. Additionally, I use teamwork skills to collaborate and consult with the members of the healthcare team such as

physiotherapist and occupational therapist to provide the best care to my client.

S.M.P. : I know that everyone has their role to play from the PSW, to the nurses to the physiotherapist they work together to provide the best care to

the client. I respect all these roles because they are all important (College of Nurses in Ontario, 2014 ). Therefore, by working on the unit I have

interacted with everyone involved in my client's care meaning I came into contact with all the roles on the unit.

S.M.P. : An example of how I demonstrated this skill is my client was under the impression that his PT class was at 1400, however, I notice that it

was for 0900 on the registration board I reported the information to my professor then to the charge nurse then I collaborated with the physiotherapist

who did not realize she had my client on this day. I acted in the best interest of my client to investigate this situation.

S.M.P. : I integrate continuous feedback from my professor in clinical and through weekly knowledge for you assessment and feedback from my

An example of this is listening to my professor's feedback after performing wound care and incorporating it into my future practice and taking my peer

feedback into consideration and incorporating it into my practice.

S.M.P. : I create, implement, and evaluate a learning plan consistent with the Quality Assurance program (CNO) every semester. Some of the goals I

have identified for my learning plan are

1) To improve my Problem-Solving skill through continuous practice and exposure as a nurse.

2)To further develop my Time Management skill by taking advantage of the opportunity I have in my pre-grad placement to work on this skill.

3) To develop my Leadership and advocacy skills through continuous practice.

S.M.P. : I take full advantage of my placement by seeking out learning opportunities and feedback that foster professional development an example

of this is providing wound care or any other procedure to a client even if they were not assigned to me just to improve my practice. Another example is

asking my charge nurse if i can administer medication to a client with insulin or injections to better my practice.

S.M.P. : "Evidence-based practice (EBP) is the conscientious and judicious use of current best evidence in conjunction with clinical expertise and

patient values to guide health care decisions. Best evidence includes empirical evidence from randomized controlled trials; evidence from other

scientific methods such as descriptive and qualitative research; as well as use of information from case reports, scientific principles, and expert

opinion" (Titler., 2008 ).

My practice is based on theory learned in class and lab and is evidence-informed because I use my textbooks and conduct research on topics I am

not knowledgeable about. An example of this is I research medications I am not knowledgeable about before administering it.

S.M.P. : I try to keep myself updated with the changes of the health care system in society as it affects nursing. I know that the best way to be notified

of any changes as it relates to the nursing practice is by using the CNO and RNAO Best Practice Guidelines.

information I received, in theory, has better-equipped me to assess for hypoglycemia this has help me to administer insulin with a better knowledge of

how to assess an insulin overdose.

ensure I am educated about medications before administering them so I am aware of possible side effects by reporting if any conditions arise and by

reporting if I believe I am incompetent to perform a task.
