1.0 Purpose
The purpose of this document is to provide the requirements of Houston’s
contractor safety process. The goal of the contractor safety process is to ensure
that contractors and PPG associates understand all applicable regulatory, PPG,
and facility EHS requirements. Information about the contractor safety process is
to be provided to contractors performing work at the Houston plant that
minimizes the potential for accidents/injuries, fires, facility or equipment damage
or inappropriate material released into the environment during the performance
of contractor work. The goal of the contractor safety process is to ensure that
contractor employees understand and comply with PPG – Houston’s contractor
safety requirements.

2.0 Scope
This procedure applies to all contractors that will be working at the Houston plant,
excluding contractors (exempt contractors) that do not enter the plant, e.g.
grounds maintenance, delivery truck drivers. A list of exempt contractors is listed
in attachment D. Training is valid for one year from training date.

3.0 Responsibility
EHS Specialist:
 Assisting supervisors with information and materials to provide for contractor
safety training.
 Act as Program Administrator and evaluate this program on a regular basis
and update this procedure and attachments as necessary.
 Maintain all records required by this contractor safety procedure.
 Review Contractor Safety Prequalification and Contractor periodic evaluation
forms and provide feedback to the Project Leader or Facility Representative

Supervisors:
 Review the content of this manual with contractors under their supervision
and ensure that they have completed the contractor safety training.
 Enforce facility EHS policies and work procedures.

Project Leader or Facility Representative:


 Review the facility specific work procedures with contractors as appropriate.
 Enforce facility EHS policies and work procedures.
 Work with the contractors to identify unusual circumstances, such as confined
space/LOTO/utility tie-ins, etc. that require special coordination of personnel
and confirmation of procedures.
 Ensure that Contractor evaluations are completed as required by Section 5.1
and 5.2 of this procedure.

4.0. Definitions
Contractor – individuals not employed by PPG, but contracted to perform
work on-site. This includes contractors that are performing maintenance
or repair, major renovation, or specialty work. This document does not
apply to a contingent workforce.

5.0 General Procedure

5.1 Review Contractor’s Safety Programs & Performance


A Contractor Safety Prequalification questionnaire shall be completed
for new contractors. The form requests specific information on the
contractor’s EHS programs and safety data that can be compared to
industry standards. The prequalification questionnaire will be reviewed by
the EHS Specialist prior to the contractor beginning work on-site. A
contractor having an inadequate safety record or an inadequate EHS
Program will be subject to additional evaluations and requirements or may
be excluded from working at the PPG Houston facility.

5.2 Evaluation of Contractor Safety Performance and Programs


A periodic evaluation of the contractor safety performance and programs
shall be completed and documented for contractors performing work. The
evaluation shall consist of an audit of the contractor’s work site for
conformance to PPG’s contractor safety rules and sometimes an
evaluation of the Contractor’s Safety Programs. Evaluations may include
the following:
a. Verification of the Contractor’s compliance with their own and PPG’s
safety and health programs and requirements
b. Verification that the Contractor is in compliance with PPG’s Alcohol,
Drugs and Other Intoxicants policy.
c. Confirmation that the Contractor’s Employees are properly trained and
qualified to safely perform assigned jobs.
d. Contractor’s employees have been instructed in the known potential for
fire, explosion and toxic release hazards related to the work.
e. Contractor’s employees have been instructed in the applicable
provisions of the Emergency Action Plan.
f. The performance of the contractor in advising PPG of any unique
hazards presented by, or discovered by the contractor’s work.
g. That the contractor documents all required training.

Evaluation of the Contractor Safety Program will be documented using the


Contractor Periodic EHS Program Evaluation Survey.
Evaluation of the Contractor’s job site shall be documented using the
Contractor Periodic Work Site Safety Survey.

5.3 Contractor
The contractor is responsible for communicating and enforcing all
applicable safety regulations for their employees and the employees of
their designated subcontractors. Employees not adhering to these
guidelines will be removed from the site by the contractor.

The contractor shall be informed of PPG’s safety requirements prior to the


bidding process. The Project Leader or designated PPG representative
shall meet with the contractor supervisor before work begins to review all
required safety documents.

Contractors must provide Material Safety Data Sheets (MSDS) to PPG for
all chemicals brought on site.

Contractors and/or subcontractors may be required to supply a current


certificate of insurance before any job can be started. No contractor will
be allowed in the plant without the proper documentation. The
maintenance manager or project leader shall maintain these documents.

5.3.1 Safety Manual


“Contractor Safety Manual”. Refer to Attachment A.
 Applicable sections of the Contractor Safety Manual and local
requirements shall be reviewed with each contractor prior to the
start of job.
 All service contract personnel must take and sign the quiz located
on Attachment C to demonstrate acknowledgment and
understanding of the safety requirements. Any questions missed
by the contractor will be reviewed with the contractor by the
supervisor managing the contractor for full understanding.
 The contractor and subcontractor are responsible for enforcing
safety, health, and environmental rules with all employees.
 The contractor and subcontractor are also responsible for assuring
that all work practices are performed in a manner that minimizes at-
risk exposure.

5.3.2 Safety/Security Orientation


Safety orientations must be completed at least annually or when
deviations for the safety manual are identified during the course of
the contractual work. Before beginning a job each contractor shall
complete the orientation process which includes:
a) Department specific process overview and description
b) Contents of the contractor safety manual

c) Security procedures including vehicle inspections, vehicle


parking, in-plant traffic, cell phone and camera policy, etc.
d) Facility assess procedures for contractor personnel
e) Safety & health rules and requirements including safe work
procedures such as hot work, line breaking, confined space
entry, lockout/tagout, excavation, and use of power tools and
battery operated equipment in electrically classified areas.
f) Emergency procedures including evacuation procedures
g) Waste Handling requirements (if applicable)
h) Hazardous or potentially hazardous environments including
potential fire, explosion or toxic release hazards related to the
contractor’s work
i) Injury Reporting
j) Housekeeping requirements
k) Hazard Communication labeling
 Contractors are required to submit a MSDS to the EHS
Specialist for any chemical brought onto facility property
l) Environmental requirements (spills, release, permits, etc)
l) Contractors must sign “hold harmless” agreements if PPG
equipment is borrowed. The use of PPG equipment should be
avoided if at all possible. A copy of this agreement is located in
Attachment B.

5.3.3 Confidentiality Agreement


Contractors who review the contractor safety manual and may have
access, visual or other, to proprietary information shall execute a
PPG confidentiality agreement should be signed by both parties
prior to disclosure of such information. Attachment B can be used
in such situations.

5.4 The EHS Specialist will maintain a contract employee injury and illness log
related to the contractor’s work. The Contractor Injury and Illness log on
the PIRP will be used to maintain the log.

6.0 Safety Orientation


 Contractors shall receive safety orientation upon job assignment and at least
annually thereafter.

7.0 Recordkeeping
 Maintain Contractor employee injury and illness log for five years
 Safety Orientation quiz sign-off sheets (Attachment C) will be maintained in
the EHS files for duration of one year or for the entire time the contractor is
on-site, whichever is a longer year.

 Contractor survey forms (Sections 5.1 and 5.2) shall be retained in the EHS files
for a minimum of one year.

Attachment A

CONTRACTOR ORIENTATION
&
SAFETY MANUAL

All contract employees performing work for PPG must adhere in detail to the requirements and procedures
outlined in this manual. PPG or the contractor will remove and replace any contract employee who fails to
comply with these rules. The PPG representative must be notified of any additional contractor employees who
arrive on-site after the initial safety and security instruction orientation. All contract employees must complete
the “Contractor Safety Orientation” prior to beginning work and/or entrance to the site. The orientation must be
completed annually. PPG General Terms and Conditions shall apply to all contractor work and may be
supplied by the PPG representative. The contractor and the contract employees shall comply with all federal,
state, and local safety regulations.

General Safety & Health Rules and Requirements


1. The contractor and/or subcontractor are responsible for the conduct of their employees and shall enforce
good order and conduct at all times. Horseplay is not permitted.

2. Work shall not be performed under or immediately adjacent to loads being hoisted and all loose items of
equipment shall be secured from falling.

3. No riders are permitted on moving equipment, rigging, or loads.

4. Acetylene, oxygen, nitrogen and other compressed gas cylinders must be securely fastened in an upright
position.

5. Jumping from docks, trucks, or other platforms and running is prohibited.

6. Do not use packages, drums, or other items in the place of ladders.

7. Lockout/tagout procedures must be used when work is performed on machinery or equipment that can
be operated.

8. While overhead work is being performed, the area below must be roped off or other equivalent measures
taken to protect workers in that area.

9. Excavations:
a. Before starting any excavation, the contractor must consult with the PPG representative from
engineering to determine the location of underground utilities such as conduct, pipelines or
tanks.
b. Necessary temporary railings and barricades of substantial construction shall be erected by the
contractor. Excavation areas shall be well marked at all times.

10. Food and beverage items are not to be carried into or consumed in the plant. Eating is only permitted in
the break room.

11. No PPG products or other PPG materials are for sale. Solicitation of any type is prohibited.

12. Cameras are not permitted on plant premises unless previously approved.

13. Fire doors must not be blocked, fastened open, or wires, cables, hoses, etc. passed through a doorway
that would prevent a fire door from completely closing.

14. Safety showers, fire extinguishers, and similar emergency equipment must be available at all times and
kept free of material, equipment or other blockage.

15. The use of or the possession of alcoholic beverages or drugs is not permitted on PPG premises.

16. The contractor is responsible for the conduct of his/her employees; fighting, gambling, theft or other
inappropriate behavior is not allowed.

17. Horseplay is not permitted and obscene or abusive language or any form of harassment will not be
tolerated.

18. Report any malfunction or potential safety hazard to the PPG representative.

19. The contractor is responsible for noting the clearance under pipe racks, conduit racks, doors, ramps, and
other such overhead obstructions and reconciling any possible interference with equipment.

20. Contractor personnel shall not connect to, adjust, or otherwise tamper with PPG equipment, unless
authorized by the PPG representative.

21. Any use of equipment, change of work methods, or movement of PPG material or equipment not
described by the PPG representative at the onset of work shall be cleared prior to performance of the
work.

22. All equipment and accessories supplied by PPG or purchased by PPG for installation are the property of
PPG. Excess materials shall not be removed from the Plant without written permission from the PPG
representative.

23. Smoking is prohibited everywhere within the plant grounds except the designated outside smoking areas.

24. Contractor employees who must work in any production, maintenance, and warehouse or laboratory area
will be required to wear safety glasses with side shields and safety shoes. Additional PPE requirements
will be communicated by the PPG representative.

25. Fall protection will be required for work performed at heights greater than 6 feet from floor level including
when working on aerial lifts. Provision of fall protection and use enforcement is the responsibility of the
contractor and/or subcontractor.

26. Do not use packages, drums, or other items in the place of ladders.

27. Cell phones, radios, beepers or other electronic devices are not allowed in the facility unless they are
intrinsically safe. To use even an intrinsically safe phone or beeper you must first have written permission
from PPG representative.

Alcohol, Drug & Other Intoxicants Policy

All contractors must comply with PPG’s alcohol, drug, and other intoxicants policy (below) while working on
PPG premises (*they must comply with the policy exactly as PPG employees do).

Statement of Policy
The unlawful possession, use, distribution, sale, dispensation, purchase or manufacture of drugs by Company
employees*(contractors) while on Company premises or while engaged in Company business is strictly
prohibited. An employee*(contractor) who reports to work with illegal drugs in his or her system or who has a
positive drug screen is in violation of this Policy. This also includes, but is not limited to, the operation of a
Company owned or leased vehicle.

Any employee*(contractor) who reports to work or engages in work with prohibited concentrations of alcohol in
his or her blood is in violation of this Policy. This also includes, but is not limited to, the operation of a
Company owned or leased vehicle. Management may prohibit the possession of all alcohol on Company
premises at designated locations.

Any employee*(contractor) who receives a conviction under any criminal drug statute for a violation occurring
on Company premises must report the conviction to the employee’s supervisor within five days after such
conviction. Failure to do so is a violation of this policy.

Hazardous or Potentially Hazardous Environments


1. PPG Industries uses a large variety of chemicals in its process to manufacture paints and resins. Many
of these materials are considered hazardous and can cause injury or illness if a person were to be
exposed. Treat all materials as hazardous and avoid contact with chemicals. If a chemical does contact
your skin, wash it off with water immediately.

2. Electrical, mechanical, pneumatic, and hydraulic equipment must be properly locked and tagged out prior
to the onset of work. The PPG representative must be notified before any electrical equipment is shut off
and/or locked out. Requirements of the LOTO SOP must be followed.

3. Opening of process piping requires special safety considerations. Piping must be locked and tagged out,
and properly drained and vented. The PPG representative must be consulted prior to beginning work.

4. Under no circumstances should a contract employee enter a tank or confined space without first having
obtained a Confined Space Entry Permit from their PPG representative.

5. Before starting any excavations, the contractor must consult with the PPG representative for all known
underground utilities or other pipe lines.

6. All hazardous outside pits, trenches, excavations, etc. must be barricaded. Excavations greater than 5
feet in depth must be properly sloped or shored to prevent wall collapse.

7. All hazardous openings or excavations must be barricaded at all times unless the work requires its
temporary removal. The area will be lighted one hour before sunset until one hour after sunrise.

8. While overhead work is being performed, signs must be posted in a conspicuous manner “Danger – Work
Overhead”. If sparks from cutting or other materials are falling, the area below must be roped off
completely.

9. A special written “HOT WORK” permit will be required for each specific job requiring the use of open
flames or spark producing equipment. These permits are for one shift only and a new permit must be
issued each shift. The permit must be conspicuous displayed at the job location. This permit is NOT
permission to smoke. The contractor must supply at least one Class ABC fire extinguisher per burning
and welding station.

10. Use of explosives must be reviewed and approved by PPG prior to use.

Security & Plant Access Procedures


1. Contract employees enter through the office entrance where they must sign in and out at the front
entrance every time they enter or exit the facility. The Maintenance Manager and/or Production Manager
will track contractors that have completed this orientation and are responsible for ensuring that all
contract employees admitted orientation was within the last 12 months.

2. Special arrangement must be made with the PPG representative for access beyond the normal work day,
on weekends, or holidays.

3. The contractor’s employees will be permitted access only to those areas necessary for the performance
of contractual work. Under no circumstances are the contractor’s employees allowed to tour or roam
around the facility or to enter areas not necessary for the performance of their work.

4. No personal vehicles are allowed inside the fenced in area of the facility unless cleared by the PPG
representative. All closed type vans, car trunks, tool boxes and other contractor vehicles and equipment
are subject to inspection by a duly authorized PPG representative while on company premises.

5. Contractor employees may park in the facility parking lot while working at the facility.

6. The contractor shall observe a facility speed limit of 5 MPH when moving any type of vehicle outdoors.

7. Contract employees are permitted to utilize plant restrooms and break rooms. Contract employees
should avoid utilizing these facilities during times that coincide with normal plant breaks.

8. The possession of any type of weapon on PPG property is prohibited.

9. Cameras are not permitted on plant premises unless previously approved. This includes cell phones or
other portable devices that are equipped with a camera.

Environmental Requirements
1. All personnel are responsible for preventing spills or releases. Contractors should utilize proper
procedures, as instructed by the PPG representative, to prevent any accidental releases of materials into
the environment.
 All containers must be stored and handled in manner which will prevent leaks or spills from contacting
or entering the land, air, ground water, storm or sanitary sewer system. Containers must be tightly
covered/closed when not in use.

2. Contractors should report any spills immediately.

Flammable & Combustible Liquids


1. Flammable liquids must be stored inside an approval (UL) metal safety can. The can must be clearly
labeled to identify the contents.

2. Bonding and/or Grounding is required whenever flammable or combustible liquids are transferred between
two containers, from a pipe outlet into a container, or during liquid agitation.

Injury Reporting
1. The treatment of injuries sustained by the contractor and/or subcontractor employees shall be the
responsibility of the contractor.

2. First aid for serious injuries may be provided on a one-time basis at the plant first aid station. The
contractor and/or subcontractor should have available emergency treatment procedures and emergency
medical telephone numbers.

3. All OSHA recordable injuries or illnesses sustained on PPG premises must be reported, in writing, to the
PPG representative and Plant Manager with in 24 hours of the occurrence.

4. PPG must be notified of any accident that damages PPG equipment or real property.

Personal Protective Equipment


1. The contractor will be informed of PPG Houston’s requirements in all manufacturing areas. In general,
safety glasses with side shields, and steel-toed shoes are required.

2. It is the responsibility of the contractor or subcontractor to enforce the use of required personnel
protective equipment by their employees.

3. The contractor and/or subcontractor will supply personal protective equipment to their employees as
needed unless instructed otherwise, in writing, by PPG.

Waste Handling Requirements


1. Sewers, ditches, drains, sumps, and trenches are not to be used for disposal of waste.

2. Dumpsters and other normal rubbish receptacles may be used only with the permission of the PPG

representative.

3. The PPG representative will provide guidance for proper disposal of any waste generated from work on-
site.

4. Flammable and combustible wastes must be removed from the premises at the end of each working day if
the PPG representative so requires.

Housekeeping Requirements
1. Housekeeping is the contractor’s responsibility. Good housekeeping is necessary at all times in the
prevention of incidents and injuries.

2. No dumping of dry or liquid trash on the plant grounds.

3. The job site must be as clean and orderly as possible while work is being performed.

4. Should the contractor fail to remove rubbish, such removal will be done by PPG at the contractor’s
expense.

5. At the end of the work, the job site must be left in an acceptable condition. The PPG representative will
make a final inspection to determine the adequacy of the final clean-up.

Hazard Communication Labeling


1. This PPG facility has a Hazard Communication Program which keeps employees informed of the hazards
of the chemicals with which they work. If you would like to review the written Hazard Communication
Program or the MSDS for any material, ask the PPG representative.

2. Hazard warning information for finished products and some maintenance chemicals can be found on the
product warning label. Hazard warning information for raw materials and some maintenance chemicals is
provided by a safety and health index (SHIS) number assignment. This system assigns a number
between 0 and 4, relative to the hazard with regard to health, flammability, and reactivity. All materials
have either a SHIS label or a finished product warning
label.

3. At no time should you ever work with a material which you


do not understand or are not aware of the hazards. If you
have any questions about the materials with which you are
working, ask your PPG contact.

Contractor’s Tools and Equipment


1. Contractors will furnish all tools and equipment. All
equipment must be well maintained and meet applicable
standards.

2. All electrical or spark producing equipment or internal combustion engines must be approved for use in any
area. Use of tools not properly rated is strictly prohibited.

3. Regardless of the type of fuel used, all fuel-driven equipment must be shut down for refueling and refueled
outdoors and in compliance with storm water management and spill prevention plans.

4. Safety guards or devices shall not be removed from tools or equipment except for repairs and must be
replaced upon completion of repair

5. Scaffold planks, ladders, ropes, ground wires, portable tools, and all other equipment shall be inspected by
the contractor and replaced if found to be damaged or defective.

6. Portable metal ladders are not permitted for electrical work.

7. All portable electrically powered tools and equipment shall be individually grounded. Insulated tools and
use of a GFCI breakers should be used when available especially when working in wet areas. Generally
grounds for welding should be through the work piece rather than using the plant structure for ground.

8. Borrowing PPG tools and equipment is permitted in extreme cases and written authorization and
release must be completed prior to use.

Emergency Procedures
1. There are significant amounts of nitrocellulose located on the east side of the property. The tank farm
located north of the production area houses a significant amount of flammable liquid. Only authorized
contractors will be permitted to work in these areas.

2. In the event of a facility evacuation, the contractor and their employees should immediately stop their
work and secure the job into a safe position, and proceed to evacuate via the closest exit. Report to the
Main office parking area to be accounted for.

3. All unusual incidents, including but not limited to accidents, injuries, property damage, etc., should be
reported immediately to the PPG representative.

4. A 24-hour phone number will be left with the PPG representative while the contractor has workers or
material on PPG premises. Likewise, a 24-hour PPG number will be supplied to the contractor
representative.

5. If the contractor sees a fire, they should first notify a PPG representative either verbally or by pulling a fire
alarm box. If it is safe to do so and the contractor is comfortable in doing so, they may use the nearest
fire extinguisher to put out the fire. Report any fire extinguisher use to the PPG representative.

6. Fire fighting tools, extinguishers, hydrants, and hoses must not be used for anything except fighting a fire.
It is the responsibility of the contractor or subcontractor to know the locations of fire alarms, fire
extinguishers, and fire exits at the work site.

7. If it becomes necessary to turn off a sprinkler system, use a hydrant or other fire fighting equipment for
anything except a fire, the PPG representative must be informed before this occurs.

8. Any work requiring the disconnection of an active fire protection system must be planned and scheduled
to minimize the actual downtime.

9. Fire doors, exits, equipment, etc. shall not be blocked by contractor equipment or vehicles. Wires,
cables, hoses, etc. shall not be passed through doorways or wall openings if the wire, cable, or hose
would prevent fire doors from closing properly.

Attachment B

Hold Harmless Agreement


I, the undersigned contract employee, understand that my use of the below tool, hereinafter referred to as “equipment”, is
totally voluntary and that the equipment is being provided to me by PPG Industries, Inc. at my request. I hereby assume
all risks in connection with my use of this equipment; and further, release PPG Industries, Inc., its employees, and agents,
from liability for any harm, injury, or damage which may befall me, including all risks connected therewith, whether
foreseen or unforeseen. Only PPG certified personnel are authorized to operate industrial powered equipment.

CONFIDENTIALITY AGREEMENT
As an employee of ___________________________________ (“Supplier”), I recognize that pursuant to the relationship between
Supplier and PPG Industries, Inc. that hires Supplier to provide services for PPG, that I will upon approval by PPG be assigned
temporarily to one or more PPG facilities and will gain knowledge of, or develop on behalf of PPG, information relating to products,
operations and plans of PPG. In consideration of my being approved by PPG for assignment to such PPG facility and gaining access to
such facility,
1. I agree (i) To use PPG Confidential Information only for the purpose of providing services to PPG (ii) Not to disclose PPG
Confidential Information without PPG’s prior written consent to any third party, and (iii) Not to allow any third party access to
PPG Confidential Information.
2. The obligations of Paragraph (1) shall continue with respect to any PPG Confidential Information for a period of ten (10) years
from the date of gaining access the PPG facility.
3. I agree to return PPG all PPG Confidential Information, which is in my possession upon request of PPG.
4. “PPG Confidential Information” means information, both oral and written, including documents and the information contained
