The student was notified verbally on April 3, 2017 and by email on April 10, 2017.
Age Appropriate Transition Assessments and Summary
Personal Interests: Personal Preference Indicators: A Guide for Planning; the assessment
and an interview with Paulo revealed that he enjoys doing housework, watching movies,
participating in outdoor activities, playing soccer, and interning at the grocery store.

Training/Education: Woodcock-Johnson III Test of Achievement; Paulo has been identified


as having a mild cognitive disability, Autism, and minimal verbal skills. Paulo is currently
placed in a self-contained MOID classroom but does interact with the general education
population during lunch. Paulo has expressed an interest in taking a general education art
class, which will help him with improving his communication skills.

Employment: Wide Range Interest & Occupation Test (WRIOT 2); Paulo is currently
receiving employment experience at a local grocery store shadowing a courtesy clerk
during his internship class. Paulo has shown an interest and initiative at this internship and
the manager of the store has expressed a desire to have Paulo work there part-time after he
graduates.

Independent Living Skills (Optional): The Ansell-Casey Life Skills Assessment (ACLSA);
student and parent report; Paulo is currently living with his parents and does housework
and chores around the house, which, according to his parents, he enjoys.

Was invitation of an outside agency considered? Were they invited? Did they attend?
Yes, the team considered inviting WorkAbility, a local employment agency in Tucson, to the
meeting. They were invited on April 10, 2017 by email and attended the IEP meeting on 4-
17-2017.
Measurable Post-Secondary Goals
Training/Education Goal: After graduation, Paulo will attend the Transition to Employment
classes through WorkAbility to prepare for employment.

Training/Education Connected IEP Annual Goal: When asked to complete the

WorkAbility application, Paulo will successfully complete the application with some
assistance from parents and teacher by the deadline of May 01, 2018. Currently,
Paulo can independently take directions and answer yes/no questions. This goal
will be measured by completion of the application before the deadline by Paulo’s
teacher and parents.

Employment Goal: After graduation, Paulo will secure a part-time job as a courtesy clerk at
a local grocery store.

Employment Connected IEP Annual Goal: When greeted by a peer or staff at school,
Paulo will successfully reciprocate the greeting in a professional manner 4/5 times
by September 15, 2017. Currently, Paulo communicates using adapted sign language
and gestures and is very quiet. This goal will be measured by teacher observation
based on Paulo’s ability to effectively reciprocate a greeting in order to prepare him
for interacting professionally with customers at the grocery store.

Independent Living Goal (When necessary): After graduation, Paulo will participate in a
soccer league through Special Olympics.

Independent Living Connected IEP Annual Goal: When participating in passing drills
for soccer during A.P.E. (Adapted Physical Education), Paulo will successfully use
appropriate language to get his peers’ attention prior to passing the ball 8/10 times
by October 22, 2017. Currently, Paulo is able to pass and shoot the soccer ball. This
goal will be measured by the A.P.E. teacher’s observations based on Paulo’s ability to
successfully communicate his desire to pass the ball to a peer.
