2018 Outdoor Urban Market
Vendor Application
Please give a comprehensive list of the items you will sell at the Urban Market.

Rules/ Regulations:
• This form is an application to be considered for participation in the Urban Market. Filling out the application does not
guarantee acceptance.
• Market management reserves the right to refuse, reject or dismiss vendors, based on the sole discretion of Market
management
• If a requested date is full at the time of registration, vendor will be placed on a wait list.
• In order to be considered, all products must be; manufactured, grown, and/or prepared by the vendor.
• Selling privileges at the Milwaukee Public Market (MPM) Outdoor Urban Market are extended to vendors for the sale
of products/food items which do not conflict with leasing agreements of current permanent Market vendors and which
have been approved for sale by the MPM
• Vendors are required to have any and all permits and licenses needed before their indicated date. It is the
responsibility of each vendor to know which licenses are applicable to their particular operation.
• All vendors must provide a copy of their Wisconsin Seller’s Permit or Social Security Number. Market staff will provide
vendor information to the Department of Revenue in accordance with state law. For information on obtaining a
Wisconsin Seller’s Permit, please call the Wisconsin Department of Revenue at (608) 266-2776.
• Growers & food vendors must have a current license from the City of Milwaukee Health Department. For information
on obtaining this permit, contact the City of Milwaukee Health Department at (414) 286-3674.
• Upon request, artists and non-food vendors should be able to present examples of product/artwork samples to the
Market office for approval.
• Vendors are responsible for supplying tables/tents/chairs as well as all setup & breakdown of their space
• Specific vendor spaces cannot be reserved from week to week, the Market reserves the right to place vendors in
order to best serve the interest of customers and fellow vendors
• We must reserve the Market parking lot for customer use only. Off-street vendor parking will not be validated.
• If a vendor is unable to participate, it is asked that they give notice within 24 hours of the indicated Saturday.
Repeated no-shows without notice will be subject to cancelation of remaining dates.

I agree to protect, indemnify, and hold harmless the MPM and the City of Milwaukee from and against any and all causes
of actions, claims, demands, suits, liability or expense by reason of loss or damage to any property or bodily injury to any
person, including death, as a direct or indirect result of the use of any rented or occupied market space or in connection
with any action or omission of the renter who shall defend the MPM or the City of Milwaukee in such cause of action or
claim.
