Weekly Diary for Emily Sinn

Week of: February 26th – March 2nd 2018

Date: 2/26 Total hours: 8 am – 4 pm


 Collect relevant information on ICU patients prior to rounds
 Participate in ICU rounds, recommend diet order advances
 In the ICU, consult with Type 1 Diabetic Patient admitted due to Diabetic Ketoacidosis/
insulin non-compliance
 EatRightPrep Practice Test 1
 Chart on ICU patients
 Provide Renal diet nutrition education and CHF consults on telemetry and orthopedic
units; chart on all patients

Date: 2/27 Total hours: 8 am – 4 pm


 Chart on both new and follow-up patients in the ICU
 Participate in ICU rounds, communicate with other disciplines to determine the best form
of patient care
 Consult with patients in telemetry units and discuss with nursing regarding eating habits
and energy intake
 Chart on both ICU and telemetry unit patients
 Continue collecting data for PI/QM Glucometer study

Date: 2/28 Total hours: 8 am – 4 pm


 Chart on both new and follow-up patients in the ICU
 Participate in ICU rounds; prior to rounds, consult with Pt. regarding need for Diabetes
education upon her transfer to telemetry unit
 Consult with patients in telemetry, oncology and orthopedic units
 Provide Low Sodium diet education for CHF, Low Fiber diet for Diverticulosis and CHO
control diet for Pt. with both CA and DM
 Complete nutrition evaluations and nutrition education documents for all Pt.’s consulted
with today
 Present PI/QM Glucometer data collected thus far to Clinical manager (data collection
will end 3/1)

 Chart on both new and follow-up patients in the ICU
 Participate in ICU rounds
 Consult with patients in telemetry and orthopedic units
 Provide Low Sodium diet education for CHF, Low Fiber diet education for a patient with
abdominal pain and CHO control diet for patient with DM
 Chart on all patients seen today
 HomeCare visit at 1 pm for weight management 5 years post gastric bypass surgery;
morbidly obese woman with DM and multiple other health complications
 Finalize data collection for PI Glucometer study
