Jonathan Leach (Chief Operating Officer ‘Chiago O'Hare Intemational Airport 10510 West Zemake Road Chicago, IL. 60666 Dear Mr, Leas This leteris in response to four separate winter operations events the Office of Alport has Jnvestignted pertaining to the 015-2016 snow season at Chicago O" are Inerational Aieport (EIR #301 5GL800033, EIR #2016GL800029, EIR #2016G1800041 and EIR #2016GL8000S1), [Below is a brief summary ofthe incidents, followed by Federal Aviation Administaton’s (FAA"s) assessment of our investigations asi pertains to 14 CFR Part 139 compliance, 1 On December 28, 2015 the following sequence of events occurred during inclement weather at Chicago O'Hare Internationa Aieport ‘© A11504 hours local time, a Boeing 777W, (Air Nippon 235) departed Runway 10L and lost single pat ‘AL 1616 hours local time, a Boeing 737-8, (VAL 345) landed Runway 1OL and lost landing light ‘© AL1619 hours local time, a Boeing 777W, (IAL 9) departed Runway 10L and lost several sirerat parts ‘© 11620 hours local time, a Boeing 737, (SWQ 2336) aborted takeofT on Runway LL, the investigation determined that FOD Was struck on takeof ol, Runway 1OL was being used for mixed operations, arrivals and departures. At 1527 hours local time, Runway 10L was inspected and airfield conditions were updated by a Chicago Deparment of Aviation (CDA) Aisport Operations Supervisor (AOS) during an active winter ‘vent with rapidly changing conditions. This inspection was an assessment of runway ‘Surlace conditions related to winter operations, when continuous monitoring was in effec. Between 1549 hours and 1620 hours local time, eight consecutive aireraft reported Runway TOL. braking ction 35 “poor”. As per the CDA's approved Snow and lee Control Plan (SICP), two consecutive poor braking action reports required the airport fo conduet a runway sesessment as soon as possible The investigation ofthis incident identified the following 14 CFR Part 139 violations 139,101(b) which requires ceriicate holders “adopt and comply sith an Aieport, entiation Manual.” * Continuous monitoring, in accordance with CDA"s approved SICP will be conducted bby monitoring the Air Traffic Control (ATC) frequency fr pilot braking action reports.
